Job Description JOB TITLE: CASE MANAGER DEPARTMENT: SERVICES REPORTS TO: SENIOR CASE MANAGER FLSA: NON-EXEMPT LOCATION: JAMAICA, NYC SALARY: $40,000.00 - $45,000.00 SUMMARY: The Case Manager assesses clients’ needs, plans a course of action, links clients to services, and monitors the progress of clients and residents in their program. Job Description Summary: Case Managers provide comprehensive services to individual clients and families residing in cluster apartments. The CASAC II (Substance Abuse Specialist) is part of this multi-disciplinary team and is responsible for delivering intensive in-home and community-based services to individuals living with severe mental illness, whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services.
