The CASAC-T Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC-T Counselor is also responsible for referrals and coordination of care in consultation with other professionals. The Justice Center mobilizes law firms, corporate legal departments, and other legal institutions to provide pro bono legal services, educates the public on pertinent legal issues, fosters strategic community relationships, and impacts public policy. The City Bar Justice Center annually provides high-quality, free civil legal services benefiting over 27,000 New Yorkers who lack the resources to hire private counsel. The Social Worker will serve individuals who are mandated to Felony Alternative to Incarceration (Felony ATI) programming through the Brooklyn Supreme Court. Reporting to the Clinical Director, the Social Worker is responsible for conducting psychosocial assessments, linking clients to a range of wrap-around services, providing comprehensive trauma-informed case management, compliance monitoring, and reporting for clients referred to the Brooklyn Mental Health Court and Brooklyn Felony ATI programs. About Secret Garden One of Barrier Free Living’s longest-standing programs is the community-based domestic violence program, otherwise known as Secret Garden, which provides case management, safety planning, occupational therapy, short and long-term individual trauma-informed counseling, advocacy within the medical, mental health, child welfare, law enforcement, and criminal justice systems, and referrals to outside services. The Brook permanently houses 189 residents; the majority are formerly street homeless and have serious mental and medical conditions (including HIV/AIDS) and substance misuse challenges. Social Workers are integral to our team and are responsible for helping chronically homeless individuals transition into permanent housing with supportive services. Our Social Workers provide direct clinical services to residents who have more intense psychiatric and behavioral health needs. The Brook provides a high level of support to its Social Workers through weekly individual supervision, rounds, on-site trainings, and staff meetings. The Social Worker will conduct these assessments and complete these treatment plans according to a strict timeline, ensuring that the process is completed as quickly as possible. The goal is to provide information that will enable defense attorneys, prosecutors, and judges to resolve cases as quickly as possible, significantly reducing the time people spend in pretrial detention. Program Overview The Cornerstone Case Management Expansion consists of a case-management approach, which incorporates a Social Worker, a Navigator, and the development of Peer Support Leaders into the Van Dyke Cornerstone Community Center. The Social Worker and Navigator will create connections to other City agencies and providers and act as “expediters” for resources such as health, mental health, employment training, housing, food, etc., to help young adult Participants (ages 18-21) with issues identified in their Individual Service Plans.
