Job DescriptionSalary: $105,000 -$110,000 JOB TITLE: WOUND, OSTOMY AND CONTINENCE NURSE (WOCN) Responsible to: Director of Patient Services WOC NURSE DEFINITION/DESCRIPTION: As per the NPUAP, wound, ostomy, and continence (WOC) nurses are experts in caring for patients with wounds, ostomies, and incontinence. This includes people with abdominal stomas, wounds, fistulas, drains, pressure injuries, and/or continence disorders. WOCNs can serve as consultants, educators, researchers, and provide patient care. The WOCN coordinates treatment to provide comprehensive wound assessments and assists to set up plans of care including interventions and goals, implementation of treatment, evaluation and outcomes for patients with wounds, fistulas, drains, ostomies or continence disorders. They act as a resource person for clinical staff and physicians and can participate in the quality improvement plan/activities for the agency. JOB DUTIES/RESPONSIBILITIES/KNOWLEDGE: • Perform wound assessment, reassessment, and management for wound and ostomy patients as requested by agency staff per policy., • Provide oversight of wound care provided by RNs and LPNs in management of acute and chronic wounds. Provides in-services on wound prevention, wound care ostomy care and incontinence related care., • Knowledge of anatomy, physiology and disorders of the skin, gastrointestinal and genitourinary systems., • Utilize the most up to date wound management theory (WOCN and NPUAP standards) when providing care to patients with acute and chronic wounds., • Formulate a teaching plan based on identified learning needs, educate patients, families and nurses on wound care, infection, and injury prevention, evaluation of patient's response to treatment and effectiveness of treatment plan., • Evaluate effectiveness of learning for patients, families and employees as appropriate., • Provide and instruct on evidence-based tools for staff nurses for decision support, • Assists clinical management to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes, wound prevention, wound care, ostomy care and incontinence., • Educate the individual/family/caregiver on how to change, empty, and manage the pouching system; normal function of the ostomy; normal characteristics of the stoma and peristomal skin; and signs of complications to report to their healthcare provider, • Treat stomal and peristomal skin complications, • Navigate resources for patient supplies to reduce ED visits, • Consult with clinicians to provide follow-up care post-discharge via review of photos of affected site(s), Telehealth, or in person visits, • Serve as a liaison between physicians, patients and manufacturers of clinical products, knowledgeable of clinical products currently available on the market., • Operates all equipment correctly and safely., • Perform focused assessments including relevant history to identify risks, contributive factors, and reversible causes on incontinence, • Advocate for reimbursement of supplies, services, and access to long-term support JOB PERFORMANCE: • Demonstrates ability for perform quality and effective services, • Demonstrates ability to set and meet objectives, and find increasingly efficient ways to perform task, • Completes work, and documentation with accuracy and within agency time frames, • Documentation meets current standards and practices, • Requires minimal supervision and is self-directed, • Understands the delivery of service within home care., • Participates in professional societies. The Clinical Case Management Specialist will ensure a smooth transition for Adult Home Residents moving into Supported Housing via the Impacted Adult Home Initiative//Settlement. Attend care coordination meetings, collaborate with ICL in-reach staff and external care coordinators, assess consumer needs, and discuss/advocate for needed services. Our Queens ACT team provides an integrated set of evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multidisciplinary mental health treatment team. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The CASAC II (Substance Abuse Specialist) is part of this multi-disciplinary team and is responsible for delivering intensive in-home and community-based services to individuals living with severe mental illness, whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services. RESPONSIBILITIES: The CASAC II will utilize a person-centered planning process to provide rehabilitation, case management and support services to individuals served by the ACT Team. The CASAC II will lead the team’s integrated dual diagnosis treatment (IDDT) with the tasks of other team members. The CASAC II will conduct assertive outreach and engagement, use motivational interviewing and stage-wise treatment interventions that target an individual’s readiness for change. Duties of the CASAC II include, but are not limited to: • Responsible for providing direct treatment and support services to clients with psychiatric diagnoses, co-occurring substance use, chronic physical health problems and complex social needs., • Conduct weekly visits to clients to assess health status., • Assess and provide direct services to participants in the following areas: health and wellness, housing, income support, education, vocational training, employment and social supports, medication support. Complete the ACT Comprehensive Assessment and lead the assessment of the substance abuse related components of assessment., • During the assessment obtains information: a) to establish the degree risk for substance use and associated harmful behaviors, and interventions required to promote progress towards recovery, and b) lead the ACT team in daily meetings and weekly clinical meetings around substance abuse issues and incorporate findings into service plans., • Identify treatment individuals should receive based on severity of substance use diagnosis, stage of change and immediate needs; recommend treatment objectives for the individual’s service plan., • Use motivational interviewing and stage-wise interventions to provide mental health, relapse prevention, harm reduction and substance abuse counseling., • Provide integrated dual diagnosis treatment (IDDT) to individuals served. Conduct weekly IDDT treatment group; run substance use support and Relapse Prevention groups. • In conjunction with Psychiatrist or Psychiatric Nurse Practitioner, provide Medication Assisted Treatment (MAT) options to treat substance use, prevent overdose and sustain recovery., • Provide individual and family therapeutic support around substance use, relapse prevention, psychoeducation, and harm reduction based on motivational interviewing and cognitive behavioral responses to substance use.
