Functions independently as a certified nurse practitioner for a patient population in collaboration and consultation with a licensed patient care team physician. Practices in accordance with a written or electronic practice agreement. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education., • Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient’s cultural background, level of understanding, personality and support systems. Serves as patient advocate., • Patient management includes the following:, • Writes admission, transfers and discharges orders., • Orders and interprets appropriate laboratory and diagnostic studies., • Orders of appropriate medication and treatments., • Refers patients for consultation when indicated i.e. dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc., • Documentation through in-depth progress notes and summaries., • May perform invasive procedures independently upon the completion of documented competency., • Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team. Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs., • Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary. Job Description POSITION SUMMARY: The Gastrointestinal Endoscopy Nurse specializes in assisting during GI procedures, providing excellent care before, during and after the procedure. The nurse will be responsible for patient monitoring and circulating during the procedure. The role includes on call duties, and scrub experience is a plus. Our interdisciplinary team provides evidence-based and high-quality care primarily to adult post-surgical patients with weight loss issue, gastrointestinal (GI) cancers, and complex non-cancer GI-related illnesses. Our surgical oncology physician leaders are under the Surgical Oncology Division. This division is affiliated with the Lineberger Comprehensive Cancer Center. These physician leaders are nationally renowned for their expertise in improving colorectal, GI, and endocrine malignancy through clinical excellence, medical education, and research. Similarly, our GI physicians are known for their efforts in enhancing patient care for upper and lower gastrointestinal patients. Lastly, we are recognized nationally as a Bariatric Center for Excellence. Our patient populations require close post-operative observation, occasional remote cardiac telemetry, early aggressive mobilization, pulmonary hygiene, extensive physical therapies, IV, central line/PICC line care, administration of TPN, anticoagulation, oxygen therapy, pain management, blood product administration, intake/output and ostomy care. Schedule: • CA Based/Licensed: Monday - Friday, 9:00 AM - 6:00 PM PST, • Non CA Based/Licensed: Monday-Friday, 9:00 AM - 6:00 PM local time zone Responsibilities: • Engage in high-value Care Management outreach calls for Included Health members with the primary goal of clinically engaging high-risk and rising-risk members, fostering strong relationships and promoting proactive health management., • Make proactive calls to members once their hospital discharge is confirmed, conducting thorough intake assessments if the member is reached., • Collaborate with hospital-based case managers to understand & support members' specific discharge needs and actively encourage members to engage with our Care Management program., • Perform initial assessments encompassing activities of daily living, cognitive functions, social determinants of health (SDOH), health beliefs and behaviors, and life planning activities to develop truly holistic care strategies., • Deliver coordinated, patient-centered virtual Care Management by telephone and/or video that consistently improves members’ health outcomes., • Generate impactful care plans collaboratively with members and our multidisciplinary care team, empowering members to achieve their desired health goals., • Bachelor of Science in Nursing (BSN), • Compact Nursing License with additional Active California Registered Nurse (RN) and Illinois License in good standing., • 5+ years of experience in clinical nursing, • 2+ years of experience working in care management, case management, and/or disease management, preferably within a health plan, health navigator, or third-party administrator (TPA) environment., • Remote care or case management experience, • Comfortable discussing a wide variety of medical conditions and experienced working with populations across all age ranges. $33hr to $40hr Exact compensation may vary based on several factors, including skills, experience, and education. This person will be working with one of Houston’s premier orthopaedic surgeons taking care of a large patient base. At Orthopaedic Associates, the Physician Assistant (PA) functions as an advanced practice practitioner. PAs maintain and promote the health of our patients, practicing medicine under the supervision of a licensed, Board Certified Orthopaedic Surgeon. Our PAs provide diagnostic, therapeutic, surgical and preventative care, under the supervision of a physician. The PA examines, diagnoses, and helps formulate treatment plans for patients.
