Job Description Summary: Case Managers provide comprehensive services to individual clients and families residing in cluster apartments. Services provided by Case Managers include assessment, case planning, developing an Independent Living Plan, case facilitation and management, counseling, referrals, information workshops and support groups. The Case Managers coordinates with other program staff and local agencies to provide short and long term care necessary to meet the needs related to moving families to permanent housing. Position Overview The Academy Charter School Dean of School Culture understands and promotes the school’s mission, vision, objectives, and policies both within and outside the immediate school community. The Dean will demonstrate the qualities expected of all academy teachers in the areas of curriculum, instruction, assessment, classroom management, professional responsibilities, professional relationships, family involvement, and school community. Assists assigned individuals in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services.
