SHPP housing specialists work with seniors who have active cases against them in Bronx Housing Court, helping them avoid eviction in both the short term and long term. We assist seniors in navigating housing court, securing funds to pay back rent, identifying eligibility for benefits, and accessing budget counseling. Job DescriptionDescription: Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Domestic Violence & Trauma Services Program offers a comprehensive suite of services to survivors of domestic violence with enhanced focus on education and linkage to health & wellness through both individual and group counseling supportive services. CHASI works in collaboration with the Staten Island Family Justice Center to expand reach and capacity. Domestic violence and trauma services are co-located. Services are provided at two different locations, 26 Bay Street Site and the Staten Island Family Justice Center (FJC). The Intensive Case Manager will manage a high-risk caseload with a specific focus on working with complex, intersectional cases of domestic and gender-based violence including, but not limited to, family violence, elder abuse, human trafficking, and sexual violence. The Intensive Case Manager will navigate clients through the many complex systems called into action in service plans, such as behavioral health services, medical services, criminal justice, family court, and child protective services. The Intensive Case Manager will be a strong advocate who can provide ongoing support to clients with the police, prosecutors, housing, shelters, schools, public assistance, and other service providers. The Intensive Case Manager will provide on-going and enhanced follow-up with clients to ensure that clients are accessing and successfully connecting with the array of supportive services available at the FJC and through off site partnerships and linkages, creating positive relationships with on and off-site partners. Job Summary: The Case Planner provides comprehensive case management services for all youth who reside in their respective Site i.e. Non-Secure Placement or Limited-Secure Placement facility and their families. The position is demanding and highly interactive with the youth and their families, staff and community. Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin. The Case Planner will maintain a caseload of 15 youth both in the facility and in the community on aftercare. The role of the Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Job Description Summary: Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach. Under this umbrella, Case managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Job Description This role requires bilingual fluency, both written and spoken. Position Overview: The Case Manager will provide ongoing on-site services that are culturally competent in the pursuit of supporting asylum seekers to resettle without needing to rely on city’s provided housing. The case manager will play a crucial role in meeting the needs of families in the sanctuary hotel site including assisting in the relocation of families. Case managers will work with individuals and families facing various challenges, such as economic hardship, mental health issues, or child welfare concerns. Position Qualifications: Minimum of Bachelor’s degree in social work, criminal justice, human services or equivalent from an accredited college as well as a minimum of two years’ experience working with youth and or families that are considered immigrants, refugees or other vulnerable populations. Shelter and or residential experience required. Must be a team player and have the ability to work independently. Must be flexible and able to work some evenings and weekends. Schedule may be staggered. Bilingual required; must have strong conversation and writing skills in both English and Spanish. Job Summary: To provide intensive clinical case management services for families affected by parental and/or child mental health and/or substance abuse, trauma, or intimate partner violence issues that place the family at risk of foster care placement. The Case Manager ensures that clients receive appropriate information and referrals to obtain and maintain housing. DUTIES & RESPONSIBILITIES: • Conduct intake and assessment process to orient client to the program and develop individual service plan reflecting assessment needs and goals., • Meet weekly with clients to assist with the achievement of individual service plan goals., • Coordinate services with Housing Specialist, other staff, and external partners, • Provide advocacy for individual clients around entitlements and services., • Provide crisis intervention and supportive counseling to strengthen clients’ ability to make appropriate life decisions., • Document case management activities in accordance with agency and funding source guidelines and procedures., • Complete Housing applications (both written and computer-generated) for clients based on their individual needs and functioning., • Achieve monthly placement quota, as assigned., • Facilitate workshops, groups, and escort clients as applicable., • Attend staff meetings and trainings to enhance skills that are related to job function., • Must be able to work flexible, alternate schedules, which may include evening and weekend shifts., • Perform other related duties.
