Job Description Job Title: Licensed Practical Nurse (LPN) – Primary Care Job Description This role offers an opportunity for a caring, dedicated, and detail-oriented Licensed Practical Nurse to join a primary care team within a Federally Qualified Health Center. The organization delivers primary healthcare, dental, and mental health services to children and adults in the Greenwich and Stamford areas, with a strong focus on individuals and families living in or near public housing. The LPN supports ambulatory primary care and specialty clinics by coordinating care, promoting continuity of care, and ensuring a safe, efficient, and patient-centered clinical environment. Responsibilities • Provide compassionate, patient-centered nursing care in an ambulatory primary care setting for children and adults., • Support specialty clinics and programs by addressing barriers to care and promoting continuity of care through proactive patient engagement., • Perform care management and care coordination activities to help patients navigate services and follow care plans., • Handle clinical communications, including incoming phone calls and electronic messages, and escalate patient concerns to the RN or provider as appropriate., • Assist with clinical tasks and procedures within the LPN scope of practice, demonstrating expertise in clinical testing and patient assessment support., • Ensure proper functioning of medical equipment by performing basic checks, reporting issues, and coordinating maintenance as needed., • Manage clinical supplies, including monitoring inventory, restocking exam rooms, and ensuring availability of necessary materials., • Prepare and maintain exam rooms so they are clean, orderly, and compliant with infection control protocols before and after patient visits., • Apply knowledge of infection prevention and control practices in all aspects of patient care and clinical operations., • Participate in quality assurance and population health initiatives, such as UDS and HEDIS, to monitor and improve service quality and operational efficiency., • Maintain a neat, organized workspace that supports efficient clinical operations and a professional patient experience., • Ensure strict adherence to HIPAA, OSHA, CLIA, HRSA, and Joint Commission guidelines in all clinical and administrative activities., • Respect patient autonomy and dignity at all times, fostering trust and demonstrating cultural sensitivity in all interactions., • Communicate clearly and professionally with the healthcare team, including providers, RNs, and support staff, to coordinate safe and effective care., • Document care and communications accurately and promptly in the electronic health record system, including Epic when applicable., • Follow organizational policies, procedures, standing orders, and clinical protocols consistently., • Collaborate in team meetings and quality improvement efforts to enhance patient outcomes and clinic workflows., • Perform other related duties as assigned to support patients, colleagues, and clinic operations. Essential Skills • Completion of an accredited Licensed Practical Nurse (LPN) program., • Current, active LPN license in the state of Connecticut., • Current CPR/BLS certification., • Minimum of 2–4 years of related clinical experience in an ambulatory healthcare setting., • Demonstrated experience in primary care, ambulatory, or acute care environments., • Knowledge of infection prevention and control practices and ability to apply them consistently., • Strong communication skills, both verbal and written, for effective interaction with patients, families, and healthcare team members., • Strong organizational skills with the ability to manage multiple tasks and priorities in a busy clinical setting., • Strong interpersonal and customer service skills, with a focus on providing a positive patient experience., • Ability to handle clinical communications, including phone and electronic messages, and escalate concerns appropriately., • Ability to maintain strict confidentiality and comply with HIPAA regulations and other regulatory standards., • Ability to work effectively in a primary care team and support specialty clinics and programs. Additional Skills & Qualifications • Experience working in a Federally Qualified Health Center or similar community-based healthcare setting., • Experience with care management and care coordination, particularly for populations facing barriers to care., • Familiarity with Epic or other electronic health record (EHR) systems., • Experience in pediatric and acute care settings., • Understanding of quality assurance and population health initiatives, including UDS and HEDIS measures., • Knowledge of and comfort working within guidelines such as OSHA, CLIA, HRSA, and Joint Commission standards., • Bilingual skills in Spanish and English, enabling effective communication with a diverse patient population., • Demonstrated commitment to serving underserved communities and addressing social determinants of health.Job Type & Location This is a Contract to Hire position based out of Greenwich, CT. Pay and Benefits The pay range for this position is $31.25 - $38.50/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
