Job Description Position Overview: The Westchester Therapeutic Foster Boarding Home (TFBH) Social Worker II will be an integral part of the Westchester Therapeutic Foster Boarding Home program. The Westchester Therapeutic Foster Boarding Home Social Worker II will be providing comprehensive case management services to youth and families involved with the Westchester County Department of Social Services. The Westchester TFBH Social Worker II will work with assigned youth and foster/kinship families to assess as well as formulate service plans to help youth achieve their permanency goals. The Westchester TFBH Social Worker II will also support and monitor foster/kinship placements to assure the safety and needs of the youth are being met. The Westchester TFBH Social Worker II will assist in the development and growth of the Westchester TFBH program. Position Qualifications: Master’s in Social Work required. Minimum 3 years of experience in child welfare, casework and foster care, or mental health; 4+ years of experience preferred. Bi-lingual (Spanish/English) preferred. Job Description Position Overview: The Mental Health Clinician (MHC) is responsible for providing the clinical support on a Per Diem basis for high-risk cases within the Sanctuary Runaway Homeless Youth (RHY) program. The MHC will also provide service to youth and families in the community who contact Sanctuary via Hotline and the Street Outreach Program (SOP). Services will be provided in-person (at The Sanctuary or in home), and/or virtually, with a goal to de-escalate crises, develop stabilization strategies for youth/family, and prevent future crisis. MHC hours served will be a combination of established “office hours” on-site at the Sanctuary, and scheduled home visits (in-person and virtual).The Mental Health Clinician shows dedication and willingness to work alongside the program Social Worker and administrative team, supporting and conducting the therapeutic interventions to best serve the population. The MHC is expected to have a nonjudgmental approach that focuses on skill building and basic principles of behavior (reinforcement, extinction, blocking and shaping). The MHC is required to function as part of an interdisciplinary team, committing to modeling pro social behavior and building positive relationships with all residents. Position Qualifications: Master’s Degree in Social Work (MSW). One (1) year documented relevant job or internship experience preferred. Ability to function successfully on interdisciplinary treatment team. Driver’s license. Excellent written and verbal skills including ability to successfully manage multiple tasks.

The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners. Job Summary: This position is responsible for establishing trusting relationships with patients while providing support in navigating and accessing resources and engaging patients in goal-driven care. Security Level: Shared Staff - FLIPA Duties/Responsibilities: • Provide a vital link between local communities and healthcare provider by helping individuals access resources and navigate systems., • Proactively outreach and engage identified individuals in need of services, follow up or social care screening by connecting via phone calls, home visits and/or in-person visits to other settings where patients can be found., • Support deployment of NYS Social Care Network screening and referral process, • Engage directly with Medicaid individuals to administer the Health-Related Social Needs Screening Tool to identify needed areas of support., • Facilitate referrals to appropriate community resources and healthcare providers., • Collaborate with the Care Team to ensure timely follow-up and service linkage., • Use designated online referral systems and databases to track and manage client referrals., • Provide care management related to social care services, • Accurately document in electronic systems and maintain detailed and organized records in compliance with organizational policies and standards., • Work closely with the Care Team, including care coordinators and other healthcare professionals, to align to a whole person care approach., • Participate in regular team meetings and contribute insights on client progress., • Attend regular supervision, staff meetings, trainings and other meetings, as requested., • Other duties as assigned Education & Experience: • Minimum of High School Diploma or GED., • Associate's degree in human services, Social Work or other related degree preferred., • Equivalent experience in lieu of education may be considered., • Minimum of 1-3 years' human services experience.
