Job DescriptionDescriptionThe Regional Patient Navigator is a highly organized and passionate team member who will float across a designated region, supporting patients throughout their wound care journey from initial treatment through discharge and transition to home care. This non-clinical role is instrumental in ensuring continuity of care, patient satisfaction, and coordination among care teams and facilities. This position will not provide direct patient care but will work closely with patients, family members, and facility resources to facilitate best-practice care, resolve barriers, and maintain clear communication across the continuum of services. The ideal candidate for this role should have a strong understanding of medical terminology and familiarity with wound treatment processes, exceptional interpersonal skills, and a commitment to patient care.
