The supervisor is expected to meet any waiver participants prior to the completion of the Revised Service Plan (RSP) developed by a Service Coordinator under their supervision. Any visits that exceed 10 in a month will be assisted by the TLC Directors. The Supervisor will have supervisory meetings with staff on at least a monthly basis, and review and sign-off on all service plans and monthly notes. In addition, they will monitor the quarterly tracking spreadsheet to ensure proper time management by staff. A supervisor MAY maintain an active caseload of waiver participants up to 25. However, this caseload must be reduced from the maximum limit allowed in relation to his/her supervisory responsibilities. The Service Coordinator Supervisor must complete a monthly face to face visit with each TBI waiver participant on their caseload. They must meet with the participant in their home at least quarterly. Job Description Position Overview: The Brave Advisor Program will work in collaboration with YAB members and OCFS staff to lead planning efforts and activities to create a YAB experience that amplifies inclusivity from across the state and shifts creative decision making to the young adult constituents. The Bravehearts will serve as process experts and will facilitate youth development skills, activities, and planning discussions with YAB members on youth leadership. Bravehearts will oversee and supervise the YAB members, facilitate board management, recruitment, retention, and collective organizational development. While providing these concrete supports, the Brave Advisor Program Coordinator will utilize their personal lived experiences combined with training and system expertise to help YAB members develop autonomy and youth leadership skills, build positive relationships, instill resiliency, all while addressing statewide systemic and environmental barriers within the child welfare, juvenile justice, mental health systems. DUTIES & RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO: • Full cycle recruitment including job posting, identifying suitable candidates, conducting phone and in-person screenings, reference and background checks, negotiating job offers, and preparing orientation documents, • Maintain and update the Applicant Tracking System (ATS), and provide updates to applicants and hiring managers on the status of applications, • Conduct weekly New Staff Orientations and review policies and procedures of the Agency, • Provide guidance and clarification on Westhab’s policies and procedures to NYC staff., • Assist management with completing payroll and with resolving timekeeping challenges at the various sites., • Conduct investigations into staff misconduct and prepare disciplinary actions, termination letters and other correspondence as needed and work with Director of HR to execute said items., • Respond to staff concerns and work with the Director of HR in developing the best course of action to resolve challenges., • Support the AVP, Program Directors, and their designees in resolving staff concerns, • Prepare weekly staffing vacancy report, • Coordinate with staff development team to schedule and track training sessions on site., • Research and recommend training programs outside the organization., • Other duties assigned by the Director of Human Resources EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS: Bachelor degree required in business or human resources management. 3-5 years of experience primarily in recruiting, employee relations and disciplinary matters. Ideal candidate will have experience in a non-profit, social service setting and a proven track record of success in recruitment and overall HR Coordination. Ability to succeed in a fast paced, environment and exceptional communication skills and ability to interact with senior level and executive level staff regularly. Must be able to multi-task. Job Description Title: Client Care Supervisor - Health Home Plus Services Reports To: Program Director FLSA: Non-Exempt Status: Full-time Supervisory Responsibility: Team of Health Home Plus Client Care Coordinators Purpose of the Role: The Health Home Plus Supervisor oversees a team of Health Home Plus Client Care Coordinators who provide intensive case management to high-need individuals with serious mental illness and/or complex behavioral and physical health conditions. The Supervisor ensures quality service delivery, compliance with Health Home Plus standards, and the development of staff capacity to support client recovery, stability, and wellness. Essential Functions of the Role: • Provide clinical and administrative supervision to a team of Health Home Plus Care Coordinators (HH+ CCCs)., • Conduct weekly one-on-one supervisions, ensuring review of key performance metrics, caseload balance, and case complexities., • Review at least 10 notes per coordinator per week to monitor timeliness, clinical quality, billing integrity, and documentation compliance., • Offer real-time feedback and support to improve staff performance and case outcomes., • Train and mentor staff in client engagement, motivational interviewing, crisis response, recovery-oriented care planning, and system navigation., • Conduct joint field visits, client calls, and side-by-side coaching to support staff growth., • Identify training gaps and provide individualized or group support to enhance staff competency in HH+ practices., • Monitor caseloads to ensure client outreach and service requirements are met at least monthly, with frequency increasing based on client acuity., • Ensure care plans are individualized, recovery-oriented, and developed in collaboration with clients, families, and providers., • Participate in rounds and team consultations for complex cases, escalating to psychiatrists, medical providers, or legal systems as needed., • Ensure all documentation (assessments, care plans, progress notes) meets Medicaid billing, Health Home Plus, and agency standards., • Respond to internal audits, data reporting needs, and external reviews or site visits., • Support client transitions, hospital discharges, and reengagement of hard-to-reach members., • Develop and maintain partnerships with external stakeholders, including DSS, OMH, legal entities, hospitals, and housing providers., • Provide case coverage or intervention support as needed in high-acuity or crisis cases., • Champion system improvements and workflow adjustments in line with best practices and programmatic goals., • Do not settle for “because that’s the way it has always been done”; be fearless in the pursuit of excellence and achieving the needed outcomes for our clients and the agency at large., • Team and location assignments based on Agency needs., • Other activities as assigned. The Assistant Community School director will support the Community School Director in ensuring the alignment and coordination of services. DUTIES AND RESPONSIBILITIES: • Assist the Community School Director in coordinating the work of different partners within the school, and aligning the work of partners with the school faculty., • Work with the school leadership to maintain a focus on results and track progress., • Oversee the implementation of a Student Success Mentorship program by ensuring that all scheduled mentor meetings are documented and supporting success while identifying needs of chronically absent students, applying interventions, and tracking impact of interventions on a weekly basis., • Participate in weekly or quarterly Community Based Organization (CBO)/ Community School meetings that include diverse school community stakeholders., • Facilitate regular meetings with Expanded Learning Time (ELT) subcontractors to address in-school needs, teacher collaboration, and event planning., • Collect and monitor program implementation data and student data for all afterschool providers and partnerships., • Work with the Parent Coordinator to ensure that families have meaningful opportunities to participate in the school., • Assist with data collection, research and prepare relevant data to provide at meetings., • Assist with projects that support school benchmarks, including: attendance initiatives, academic support interventions, and family engagement., • Assist in the coordination of the allocation of space for student and family support services, and for special events., • Develop strong relationships with teachers, parents and students., • Other duties as assigned. EXPERIENCE AND EDUCATION REQUIREMENTS: Bachelor’s degree in Education, Social work or related field. Bilingual, fluent in Spanish preferred. Ability to work effectively with school aged youth, educators, families and communities from a wide range of cultural, social and economic backgrounds. Experience working in school based settings, with knowledge about tenants of community organizing, student support services, and youth development theories.
