Job Description About the Role The Direct Support Professional (DSP) provides person centered support based on the individual’s needs, preferences and choices. Job Summary: The goals of the Preventive case planner, within the Family Connections model, are to keep families together, while continually assessing for risk and safety issues. Through the Family Connections model, they are to help strengthen families, stabilize and maintain family functioning. Under the direction of the social work supervisor, the case planner shall serve as the primary service provider. The model requires great time flexibility, as we meet with the entire family system most of the time. Therefore, workers must make themselves available when the families are. Job Responsibilities: The Case Planner will have a caseload of 12 families. Each family will be seen for one hour of change focused intervention, primarily in the home, each week. Therefore 2-4 contacts are expected per month. Based on the Family Connections model, once a case is assigned, face-to-face contact must be made with the family to present the model and explain the expectations with 24 hours. Concurrently the JHV is set up with ACS. Once that is completed the intake is conducted, within a short timeframe, and all documents must be signed. The intake referral form must be completed and placed in CONNX within 7 days. Case Planners are to carry out ongoing risk and safety assessments of the family and clearly document this within their progress notes. Any concerns must be brought to the immediate attention of their supervisor. Case Planners must complete all Family Connections paperwork, within the required timeframes. These include the CASI instrument; Comprehensive Family Assessments; Service Plans; Evaluations for Change; and Case Closure Form. In addition, all FASP’s must be completed and submitted to their supervisor for review two weeks prior to submission to ACS. Progress notes must be written within 72 hours of the contact and inputted into CONNX. These notes must follow the template that was developed, which encompasses Family Connections, as well as ACS guidelines. Notes are to be printed out and submitted to their supervisors on a weekly basis for review. All casework contacts must be entered into PROMIS on a weekly basis. To facilitate family team conferences for your caseload and enter into PROMIS by end of week. Family team conferences will be held at the beginning of a case, or at the point of case closure. Weekly contact grids and calendars are to be submitted to supervisor at the end of each week. To participate weekly in supervision; and bi-weekly in group supervision. Job Description About the Role The Clinical Manager working in the 29i/CFTSS program for the Enhance Family Foster Care (EFFC) Program is responsible for providing an array of administrative and supervisory services within the program as well as providing limited direct clinical services for youth served by the EFFC program. What You’ll Do • Provide administrative management and oversight for the 29i/CFTSS services provided to the EFFC program including reviewing program data, managing productivity and case assignment, and helping to develop the program to best serve the foster care population., • Provide clinical supervision to all employees providing 29i/CFTSS services to EFFC children. Job Description Job Summary: Family Development Coach provides case management and related workshops. The FDC is the primary contact for fathers participating in the program. The FDC will contact each assigned participant at least once every two weeks to assess progress to achieving goals for the Individual Service Plan (ISP). Connect participants to services that address immediate needs related to housing, substance abuse, physical and mental well-being, violent or risky behavior, and other areas that threaten to inhibit full participation and ability to achieve personal and program goals. The Fatherhood Initiative will implement parenting skills training to participants with two different curricula: ProFatherhing15 and Supporting Father Involvement (SFI)., • Guided by DYCD’s Case Management Toolkit, develop goal-oriented Individual Service Plan (ISP), and facilitate applications and referrals to various services and supports. Address the changing needs of fathers and adapt services accordingly., • Provide individual, family and group counseling, provide direct services/case planning to clients including mediation/conflict resolution training to fathers to facilitate effective co-parenting and visitation. Through this dedication, the TRC aims to transform services for survivors of violent crime. Rising Ground’s TRC is designed to help victims of violent crimes overcome barriers to accessing mental health treatment, health care, and legal resources in the acute aftermath of trauma. The Clinical Social Worker/Therapist of the Trauma Recovery Center (TRC) will provide clinical intakes, assessments, planning and implementing individual and group psychotherapy, assertive outreach, and clinical case management services, using evidence-based practices that are effective for the service population. The ideal candidate will bring a passion for social justice and anti-oppressive practice, ample experience providing clinical services to survivors of trauma, and a desire to work collaboratively with a team of creative and committed advocates. All candidates must have a strong understanding of trauma-based work and have experience providing crisis intervention and emotional support to trauma survivors. The ideal candidate recognizes that “the most harmed are the least helped,” understands the importance of removing barriers to care, and has a passion to increase accessibility of services to survivors of violent crimes. This position is located in our downtown Brooklyn location with a hybrid schedule of 3 office days, and 2 days remote. To meet program needs, all staff in the TRC will conduct home visits and accompaniment/advocacy in the community, as needed. What You’ll Do Clinical Intervention, Consultation, Psychotherapy, Liaison, Outreach and Case Management: • Conducts clinical intakes of new clients using a trauma-informed approach and identifies service needs when appropriate. Assessment includes identifying presenting problems, psychosocial history, mental status exam, and DSM-5 diagnosis., • Clinical interventions include crisis intervention and emotional support to clients., • Clinical services also include short-term, trauma-informed individual and group psychotherapy as well as comprehensive clinical case management, liaison with other health care providers, and referral and accompaniment to community services when appropriate., • May visit patient at home and/or in the community for the purpose of outreach and follow-up if the patient is unable to keep regularly scheduled appointments., • Caseload consists of complex clients, all of whom have histories of trauma, including acute and chronic physical and sexual abuse, and are victims of interpersonal violence such as sexual assault, physical assault, domestic violence, gunshot wounds, political torture, and immigration trauma. Many clients also have concurrent medical problems, psychosocial problems, and may have substance abuse problems as well., • Utilizes knowledge of the mental health, medical, and community resources in New York City and maintains familiarity with new programs., • Develops a comprehensive treatment and service plan for each client, integrating knowledge from a wide range of human service resources., • Facilitates coordination of services among various agencies; advocates for and links client to needed services. Helps to eligibilize clients for social security disability, Victim of Crime, and other financial entitlements. Provides assistance with U-Visas and psychological asylum evaluations as needed. Serves as a client advocate and liaison with the District Attorney’s Office and Police Department for victims of crime wishing to file police reports, restraining orders, etc. Clinical Supervision, Teaching, Training, and Program Development: • May provide supervision to clinical interns., • Participates in in-service trainings and case conferences., • Conducts community presentations., • May assist with program development, including identification and implementation of types of appropriate measurement-guided mental health treatment.
