Program Description Howie the Harp Advocacy Center (HTH), which is run by professionals who have personal experience with the mental health system, offers an array of services that help individuals with mental health concerns find meaningful, permanent employment and develop careers, primarily in human services. All HTH programs are based on the core values of self-determination and peer expertise. Through the Comprehensive Peer Training Program, Assisted Competitive Employment Program (ACE), and other initiatives, the Academy offers a wide range of services including training, internship experience, job placement and support, and continuing education services. Position Overview The Peer Trainer will develop and offer a series of workshops designed to help peer specialists learn about different specialties in peer work and practice the skills that different roles would require. This would help better prepare those who are interested in working with people who are experiencing mental health crises in a variety of settings such as: mobile crisis response teams; in hospitals; in crisis respite residences; street outreach; or on Intensive Mobile Treatment or Assertive Community Treatment Teams (IMT/ACT), and Intensive and Sustained Engagement Team (INSET) which often work with people with higher needs who may frequently be in crisis. The target populations for these trainings would be those enrolled in our Howie the Harp peer training program as well as for people already working in the peer field who would like to explore different types of jobs or hone skills in their current areas of practice. Key Performance Indicators • Develops and annually updates a curriculum of relevant trainings and workshops that cover the latest developments in NYC crisis response services and their best practices., • Conduct trainings as scheduled., • Group notes are completed within 48 business hours of the class meeting. Team members will work collaboratively with shelter staff and use assertive engagement strategies to proactively engage individuals in services. ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature. ACT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant’s choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Overview The Licensed Practical Nurse (LPN) functions as a primary practitioner who assesses and monitors ACT participants’ clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assist individuals in their achievement of mental health, medical, rehabilitation and recovery goals. Program/Department Description The Residential Crisis Support program, or “The Respite,” is a residential mental health crisis, alternative-to-hospitalization program offering short-term Peer-led care in a friendly and supportive home-like environment. Guests are provided with the opportunity to learn and practice recovery and relapse prevention skills to support their current mental health crisis and to provide more effective tools to cope with future crises and avoid hospital-based care. Recovery-oriented services are provided 24-hours a day and include peer support, self-advocacy education and self-help training while supporting guests to maintain their established daily activities such as visiting with friends, attending a doctor or other appointments or going to work or school. Guests are encouraged to participate in the operations of the residence, including working together to prepare communal meals. The maximum length of stay is 28 days. Community Access’s OMH licensed Intensive Crisis Residence (ICR) is inspired by the Soteria model and is designed as an alternative to psychiatric hospitalization for people going through an acute psychiatric crisis and who would benefit from a respite period in a stabilizing, home-like environment with 24-hour peer support and treatment services. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. Position Overview The Peer Health Navigator’s primary focus is connecting Respite or ICR guests to community resources that can help support them as they move through the crisis and return to their lives in the community. This is a key part of successful discharge planning as it will help guests forge and sustain connections to a variety of professional and community-based supports. These peer navigation services would continue for 90 days after discharge to ensure successful crisis resolution and stability upon their return to the community. IMT team members share responsibility for the people they serve, helping individuals attain housing and other personal and safety goals that may be of a health, social, work or education nature.
