Job Description Overview The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Responsibilities • Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed., • Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures., • Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines., • Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel., • Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Qualifications Education • Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment., • Must be able to obtain and maintain CPR certification.

Job Description Liberation Programs is seeking a Full Time Nurse for our busy outpatient clinic in Stamford, CT. The shift is (approximately) Monday to Friday, 11am-7:30pm. The staff Nurse helps carry out the agency mission and duties of the medical services unit. DUTIES Methadone Outpatient Clinic • Provide “back-up” and relief for the methadone dispensing nurse, • Dose via Avatar and hand dose in emergency situations, • Calibrate equipment and close out for the day, • Document in Avatar as needed, • Document in AWARDS as needed to provide medical care coordination, • Provide patient education on medications, side effects, medical conditions., • Lead “wellness” efforts for clients QUALIFICATIONS: • Current CT Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) ., • Three to five years of direct patient care in hospital, primary care, psychiatric facility or home care. Experience in substance abuse/methadone dispensing preferred., • Knowledge of the principles of addiction treatment. Practical experience in the field of addiction treatment is preferred., • A work history that demonstrates leadership, accomplishment and the ability to get work accomplished on time and within predetermined specifications., • Computer skills, with demonstrated ability with Microsoft Outlook., • Familiarity with methadone dispensing software, Accumedic preferred. Job Description Overview Nurses work in collaboration with the interdisciplinary team to provide patients with care, education, and support. LPNs participate in the planning, implementation, and evaluation of nursing care, based on the objectives, standards, and policies of practical nursing. LPNs work under the direction of a Registered Nurse and/or Physician. Responsibilities Responsibilities • Maintains a comfortable, orderly, safe, and clean environment for patients., • Measures & records patients’ vital signs (such as height, weight, temperature, blood pressure, pulse, respirations, food/fluid input/output, etc.) in the electronic health record., • Assists with bathing, dressing, and personal hygiene of the patient., • Monitors catheters., • Dresses wounds., • Assists with patient positioning, standing, walking, and transferring., • Collect samples of blood and/or urine for testing., • Performs routine laboratory tests as ordered., • Prepares, administers, and documents prescribed medications via the ordered route., • Reports adverse reactions to medications or treatments according to the policy regarding medication administration., • Assists in relevant patient teaching., • Records all care information concisely, accurately, completely, promptly, in the appropriate format, and on the appropriate forms/templates, • Performs other position-related duties as assigned, depending on the assignment setting, • Direct care provided by unlicensed staff., • Upholds HIPAA regulations., • Floats among various clinical services, where qualified., • Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients., • Provide clinically appropriate care as directed by the Registered Nurse and/or Physician., • When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc., • Monitor patients' condition and keep it stable while en route to the clinic., • Aid in the preparation of the transfer of patients by Urgicare and/or EMS to the emergency units or hospitals., • Document the medical treatment that is given to the patient., • Keep the equipment clean after use; check and replace damaged and used supplies., • Ability to lift fifty (50) pounds., • Moving, lifting, or transferring patients may involve lifting up to 100 pounds., • Ability to stand for extended periods., • Fine motor skills. Scheduling note: Nurses are required to work every other weekend. Qualifications • Graduate of an accredited LPN program., • Licensed Practical Nurse currently registered to practice in New York State., • Demonstrated clinical competence and skills in problem-solving, communication, priority-setting, and interpersonal relationships) with experience in clinical nursing practice as a medical/surgical, behavioral health, or correctional health nursing experience preferred., • Demonstrated commitment to continuing professional development through attendance at continuing education in-service sessions and participation in professional/organizational committees/councils., • Basic Life Support Certification is required; ACLS is encouraged.
