keeps Patient Care Manager/designee and/or physician, abreast of changes in patient’s condition and/or treatments, as appropriate; uses clinical judgment in delegating assignments in providing patient care, and ensures that assignments are completed in a timely fashion. \n 9.Performs grade I-IV Decubitus Care; performs preventive skin care measures; applies simple dressings, maintaining principles of aseptic technique; applies warm and cold compresses, consistent with facility procedure; performs irrigations, consistent with Facility procedure; performs other procedures related to skin care, as necessary. \n 10.Administers medications correctly and safely; correctly identifies medication in terms of action, dosage, side effects, and implications for the patient; meets standard on medication administration examination; demonstrates preparation of local solutions. \n Administers and documents medication correctly; educates patients and significant others related to drug and food interactions; educates patients and significant others related to drug and food interactions. \n 11.Participates in patient and family education; provides patient with an explanation of his/her condition as indicated; communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; provides for early discharge planning and appropriate referrals for post-hospital care; evaluates the effectiveness of teaching by feedback from patient/ family and documents same. \n 12.Performs related duties as required. Describe where, when, and why the problem behaviors occur while supplying descriptive monthly progress notes., • Train staff and family members directly while allowing enough time for questions and discussion about the implementation of the treatment plan., • Assume responsibility for parent training, ABA (Applied Behavioral Analysis) therapist supervision, and treatment planning/BTM services for each case., • Record all session notes in the Provider Portal and finalize monthly notes on or before given deadlines (all billing and date to be finalized PRIOR TO/BEFORE deadlines set forth., • Maintain an open line and chain of communication via phone call, text and email with all teams and client’s team (guardians, schools, daycare (in form of Coordination of Care if available))., • Adherence to carrier-based templates and guidelines (ratios, code usage, limitations and data)., • Adherence to deadlines and due dates to allow for continuation of care (avoidance of gaps in authorization)., • Review your patients’ behavior data and note any discrepancies (e.g., lack of documentation, inappropriate restrictions, or lack of proper consequences) and situations in which the current behavior program or treatment approach were not followed appropriately.
