Ensure that issues/concerns encountered or raised at/by the patients are resolved; to ensure smooth operation of the medical case management, including troubleshoot any crisis with clients, grievance, complaint, etc. with patients and/or care team. • Created and maintaining up to date clinical supervision logs., • Monitor and ensure medical case management staff keep accurate patients related records on the services and activities such as assessment, care plans, patient’s interactions in designate database and reporting structures., • Monitor and ensure medical case management staff progress by tracking performance milestones, resolving problems as they arise, and coordinating status update meetings with appropriate staff., • Monitor team’s performance and ensure meeting the service goals and quota set by Apicha Department of Support Services., • Ensure the medical case management staff follows Apicha CHC policy and procedure as required., • Perform quality assurance and quality improvement activities., • Participate/collaborate with internal and external entities on quality improvement activities., • Facilitate on-going care team communication for betterment of patients., • Respond to any inquiry from external entities such as Department of Health, AIDS Institute, MCO, or other service providers on non-contractual related inquiries., • Provide information and facilitate referral to services available at the Agency, as needed., • Collaborate with internal and external entities in fostering effective partnership, • Represent the agency at various venues to promote agency services. Qualifications: • Advanced degree in related field (i.e. health, nursing, mental health, or public health) with 3 years of full-time experience in related field required. This role oversees and coaches school-based special education leaders and service providers, including Special Education Coordinators, Mental Health and Behavior Program Managers, and speech-language pathologists, while collaborating closely with school leadership to ensure strong instructional practice and compliance. The Director designs, implements, and monitors school-based special education systems in partnership with other team members. Additionally, the Director supports autism program fidelity, family partnership, professional development, assessment accommodations, and enrollment processes, creating the conditions for equitable, inclusive, and high-quality special education programming across the campus. The Doe Fund is a leading New York City non-profit organization that operates the enormously visible Ready, Willing & Able (RWA) program, whose participants clean over 115 miles of streets and sidewalks every day. With an $80 million budget, 500+ staff members, and 46,000 individual donors, The Doe Fund is nationally acclaimed as a work-based model for solving homelessness and criminal recidivism. We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs. We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals, including veterans living with a severe mental illness and/or history of substance use. The housing program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 90-unit residence will provide housing for 54 individuals referred by the NYC Human Resources Administration (HRA), and the rest 36 units will be apartments for affordable to low-income households. The Case Manager is responsible for providing supportive case management services and counseling to the residents referred by HRA. The successful candidate will have strong clinical skills and experience working with individuals diagnosed with co-occurring disorders and/or trauma. Key Responsibilities: • Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them, • Develop and facilitate therapeutic groups for residents, • Maintain resident records in compliance with agency and funding standards, • Conduct and review assessments, • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency, • Monitor and ensure residents are linked to appropriate treatment and services, • Provide counseling and crisis intervention as necessary, • Assist residents in securing and maintaining benefits, • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs, • Monitor and assist residents with rent payments and budgeting, • Conduct regular unit inspections, • Educate residents about approaches to mental health and substance use, • Compile resources on mental health information and treatment strategies, • Attend ongoing training to enhance the services provided to the residents, • Assist with the completion of reports as needed, • Other duties as assignedThe Ideal Candidate:, • You have a Bachelor's degree in Social Work or a relevant field., • You have at least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings, • You have expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques, • You have excellent clinical and case management skill,s including knowledge of benefits in the NYC Metro area, • You are sensitive to community issues and needs, • You have outstanding interpersonal and communication skills, both oral and written, • You have strong ethical standards, confidentiality, and personal integrity, • You are computer proficient (i.e., email, Microsoft Word, Excel, and PowerPoint) Submit your application for review.
