provide assistance in preparing for and facilitating conferences and home visits when needed;, • Monitor indoor and outdoor learning areas daily to observe the implementation of the daily schedule; teacher-child interactions; teachers’ performance; child supervision; and safety practices;, • Monitor classrooms a minimum of monthly to ensure the physical environment is attractive with children’s work on display, warm, nurturing, clutter-free, and conducive to child-directed play, and all emergency procedures are posted conspicuously;, • Monitor family-style meal time and CACFP meal counts to ensure accurate data is reported for reimbursement;, • Provide ongoing staff supervision, training, and professional development to address staff’s professional development needs and ensure staff are able to implement all BSECDC policies and procedures, including that every classroom staff/volunteer is aware of the steps to take in an emergency and the “Lost Child” policy a minimum of three times each year;, • Support the use of technology and data management systems by staff to promote efficiency, reliability, and accuracy in data analysis and reporting;, • Provide each teaching staff member with a minimum of two reflective supervision meetings each year; The Hospice Inpatient Clinical Coordinator is responsible for developing and strengthening business relationships with hospital partners, assessing patients referred for hospice enrollment, and providing General Inpatient (GIP) level of care to hospice in-patients with specialized clinical skills and compassion. Job DescriptionDescription: Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Heroin Overdose Prevention Education (HOPE) program provides an opportunity for individuals involved with the criminal justice system who are charged with an offense and exhibit substance use to access care and treatment. The long-term goal of the program is to support participants in managing their lives in the community and prevent future arrests and returns to incarceration. The Counseling Coordinator is a key member of the Justice Initiatives team, serving as the lead for HOPE while also supporting ATI through substance use assessments, individual counseling, crisis intervention, treatment planning, recovery support, and supervision of a small team of case managers. This role works closely with other members of the Justice Initiatives management team, including the Senior Director of Justice Initiatives and the Coordinator of Justice & Recovery, as well as court personnel, case managers, Senior Peer Mentors, and community partners to deliver responsive, participant-centered services that promote engagement, stability, and long-term success. DUTIES & RESPONSIBILITIES • Conduct comprehensive substance use disorder screenings, assessments, and evaluations for individuals referred through the Richmond County District Attorney’s Office, courthouse engagements, ATI referrals, and community-based referrals, • Develop individualized action plans and treatment recommendations using trauma-informed and person-centered approaches, • Provide individual counseling sessions and, as appropriate, group counseling using evidence-based interventions focused on substance use, relapse prevention, and recovery support, • Facilitate recovery-oriented skill building and psychoeducational group activities, • Provide crisis intervention and de-escalation support to program participants as needed, • Conduct follow-up and aftercare calls to support sustained engagement and recovery, • Coordinate referrals and linkages to treatment, ancillary, and supportive services, • Coordinate referrals and linkages to treatment, ancillary, and supportive services, while monitoring follow-through on referrals made by assigned case managers, • Participate in weekly case review meetings with court partners and other external stakeholders for HOPE and ATI, and provide case consultation, guidance, and follow-up direction to assigned case managers, • Serve as the lead for HOPE while supervising a small team of case managers who provide counseling support, service coordination, and participant engagement services across HOPE and ATI, • Complete and review required program documentation accurately and in a timely manner using Salesforce and other approved systems, • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values, • Other duties as requested or at the needs of the program and/or agency The above is intended to describe the essential job functions, the general supplemental functions, and the essential requirements for the performance of the job; it is not to be construed as an exhaustive statement of all the job functions.
