The Certified Nursing Assistant CNA is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional well-being, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures. Follows current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs) to include mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting. As assigned, performs incidental household services essential to the patient's health care at home that are necessary to prevent or postpone institutionalization. Completes a clinical note for each visit within required timeframes, which must be incorporated into the patient's record. Provides care according to the aide plan of care/assignment sheet and in accordance to State Practice Act or regulations. License Requirements • Successful completion of a competency evaluation., • Current CPR certification required., • Must be mature and able to deal effectively with the demands of a home health aide position. Job DescriptionJob Overview As a Patient Liaison for a specialty pharmacy, you will serve as a key connection between the pharmacy, healthcare providers, and patients. Key Responsibilities: * Assist physicians in delivering primary and chronic care services, including patient intake, history-taking, and follow-up coordination. * Analyze and interpret healthcare statistics to support clinical decision-making and population health strategies. * Work with patient data and payer models (private and government insurance) to assist in identifying gaps in care and ensure payer compliance. * Support chronic disease management by tracking patient metrics, preventive services, and health screenings. * Educate patients on public health topics, self-care, and chronic condition management in a culturally sensitive and bilingual manner (English/Spanish). * Collaborate in the design and execution of quality improvement and population health initiatives. * Document accurately in electronic medical record (EMR) systems and assist in producing care reports. * Understand and support the integration of socialized medicine models and public health frameworks in practice workflows. * Maintain strict patient confidentiality and adherence to HIPAA and all compliance standards. Qualifications: * Medical degree from an accredited foreign medical school. * Bilingual in English and Spanish (required). * Familiarity with U.S. healthcare systems, including insurance payers, CMS, Medicare/Medicaid, and value-based care models. * Understanding of population health, public health policy, and social determinants of health. * Strong organizational and time management skills. * Proficient in Microsoft Office, particularly Excel, and data entry/management tools. * Comfortable using EMR platforms (experience preferred). * Excellent written and verbal communication skills. * ECFMG certification preferred but not required. * Must be authorized to work in the United States.
