Reporting to the Mobile Operations Manager, the Mobile Health Patient Services Associate is responsible for working in collaboration with clinical staff and collaborative partners to provide administrative and relationship management support to Project Renewal's Mobile Health Program. Working as a member of Project Renewal's Primary Care Mobile Unit team--and in partnership with PRI based programs and external partners--the Patient Services Associate works directly on the mobile health vehicles to identify and link existing and potential patients to available health services. Essential Duties and Responsibilities: • Identify and link potential patients to available health services, including but not limited to tuberculosis screening, comprehensive exams, preventative care screenings, substance abuse treatment programs and treatment of chronic disease and behavioral health.
