Addresses issues with appropriate actions, • Collaborates and communicates with the health care team, • Integrates patient rights into work practice, • Utilizes and monitors methods of Infection Control, • Develops and participates in Performance Improvement initiatives, • Completes competency requirements as well as CERP requirements to maintain IBCLC Certification, • Attends and participates in mandatory in-services., • Attends and presents at staff meetings as required/requested, • Utilizes cost containment practices, • Adheres to all WPHC policies and procedures including dress code, code of conduct, customer service, attendance and submission of annual physicals, • Functions as a resource person regarding lactation issues, to hospital staff, physicians and patients, • Graduate from an accredited school of nursing with current New York registration., • BSN Degree required., • Knowledge of current nursing, lactation, medical and health trends and practices in rendering of patient care, • Certification from the IBCLE (International Board of Certified Lactation Examiners) with a certification of IBCLC (International Board Certified Lactation Consultant), • BLS, NRP and ACLS certification is required.

The Consultant is an integral part of the CDI team and works closely with the client’s CDI team, supporting classroom education and mentoring. They are responsible for ensuring the successful transfer of CDI best practices from the consulting team to the client team. Responsibilities: • Review inpatient medical records for identified payer populations on admission and throughout hospitalization., • Analyze clinical information to identify areas within the chart for potential gaps in physician documentation., • Formulate credible clinical documentation clarifications to improve clinical documentation of principal diagnosis, co-morbidities, present on admission (POA), quality measures, and patient safety indicators (PSI)., • Facilitate modifications to clinical documentation through extensive interaction with physicians, nurses, and ancillary staff., • Work collaboratively with the coding staff to assure documentation of discharge diagnoses and comorbidities are a complete reflection of the patient’s clinical status and care., • Develop and implement plans for education of physician, nursing, and ancillary staff on documentation improvement. Job Description Reports To: Medical Director Program: Article 32 and Article 31 Clinics Location: White Plains, NY Hours Per Week: 35 FLSA Status: Full Time, Exempt Salary Range: $86,000-$91,000 Summary of Duties: The Registered Nurse has overall responsibility of assessing and monitoring clients for gaps in preventive services and physical health conditions related to behavioral health treatment or common physical co-morbidities, in our Comprehensive Health Center, licensed by the Office of Addiction Services and Supports (OASAS), Article 32 substance use treatment program. As well as our Article 31 Mental Health Outpatient Treatment and Rehabilitative Services. The Nurse provides screening and monitoring of key health indicators and health risk, as well as clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome. The Nurse will coordinate completion of required steps for initiation of Medication Assisted Treatment (MAT). The Nurse will serve as a consultant to other team members and may take over some of the work needed to adequately coordinate care if the management of a particular medical condition becomes complex.
