Job Description JOB DEFINITION: The Program Manager assumes overall daily responsibility for the Health Home Care Management (HHCM) program to ensure that clients assigned to the program receive client-centered, integrated, multidisciplinary coordinated care that promotes better health and more appropriate utilization of health care. This position and the program will be evaluated on consumer outcomes. EDUCATIONAL REQUIREMENTS: Bachelor's degree in health profession from a program approved by the New York State Department of Education required.
