Job Description Pay: From $65.00 to 75 per hour Job description: Overview Where: High Class Home Health care Registered Nurse (RN) to visit client in their homes throughout Essex, Hudson, and Union County. Shift Type: Per Diem High Class Home Health care is seeking a New Jersey licensed Registered Nurse (RN) for in-home visits to our clients throughout Hudson, Essex, Union County . • This is a non-medical per diem position Home Care Field RN responsibilities include: • Setting appointments with High Class Home Health Care's clients in their Hudson, Essex, Union County homes during a regular High Class Home Health care shift; mileage is reimbursed per Company policy, • Creating in-home initial client assessments and care plans based on clients' specific care needs, • Conducting follow-up in-home assessments at specified intervals, • Conducting periodic telephone assessments, • Revising care plans as necessary, • Case management, • Supervising Certified Home Health Aides (CHHAs), • Prior Home Care RN experience STRONGLY preferred, • Enjoys meeting with and talking with older adults, • Prior RN experience with in-home care assessments, • Current New Jersey Registered Nurse License, • Excellent written and verbal communicator with strong interpersonal skills, • Must be self-motivated, organized, and able to work independently, • Able and willing to travel to clients' homes throughout Hudson, Essex, Union County, • Has a valid driver's license and a reliable car, willing to travel to clients' homes, • Has basic computer skills (data entry on a web-based program, email, scanning) Medical Specialty: • Home Health Physical Setting: • Acute care, • Clinic, • Inpatient, • Long term care, • Nursing home, • Outpatient, • Rehabilitation center Work Location: In person
