Job Description Overview Physician Affiliate Group of New York (PAGNY) is adding a Patient Safety Officer to the group at NYC Health + Hospitals/ South Brooklyn Health. The Hospital is part of NYC Health + Hospitals. Responsibilities The Patient Safety Officer will join the administration team at South Brooklyn Health. Responsibilities Include: • Safety Event Reporting: Track, trend and manage event reporting for “near misses” or “good catches"; implement and maintain a “good catch” celebration program., • Root Cause Analysis: Participate in or lead root cause analysis discussions; participate in performance improvement for corrective action plans, • Leapfrog: Coordinate data collection and entry for Leap Frog twice a year along with Quality department; identify and participate in performance improvement projects related to low scoring areas of Leapfrog, • Culture of Safety Survey: Organize distribution of the survey every two years; categorize the results by department and discipline to share with executive leadership, department leadership and front-line staff; track action plans, • National Patient Safety Goals: Track and trend data/improvements for National Patient Safety goals on quarterly basis at a minimum; regularly share updates in trends to executive leadership; utilize recognized/standardized tools for analysis such as run charts etc. and approved methods of performance improvement i.e. PDSA., • Patient Safety Meetings: Attend monthly System-Wide Patient Safety Council meetings; coordinate regular (monthly/quarterly) PS meetings at facility; review reported incidents, trends of national patient safety goals, good catches, and any other pertinent projects, • Patient Safety Rounding: Participate in and coordinate Executive Leadership Rounds on a regular basis (weekly-monthly); collect, track, and coordinate action plans for shared patient safety concerns; share PS improvements regularly with front-line staff, • Patient Safety Education: Complete TEAMSTEPPS training for new employee orientation or for requesting units; regularly educate staff on National Patient Safety Goals, how to report safety events and the importance of reporting, • Patient Safety Projects: Help mentor and sponsor patient safety projects and encourage submission to conferences such as IHI or for awards such as HANYS Pinnacle Award Qualifications • Certification in Safety or Quality (e.g. CPPS) preferred; if not achieved prior to hire will be completed within 2 years of start date, • Proficiency in EPIC, specifically with data mining and reporting, • Proficiency in Microsoft Office programs, • Competitive compensation package, • 10% 401K company contribution after one year of service, with 3% company contribution starting day one, • Choice of a three tiered, starting at FREE medical plans starting day one, • Excellent dental insurance, including orthodontics coverage starting day one, • Generous paid time off program, sick time and holidays, • Salary Amount: $150,000 Physician Affiliate Group of New York (PAGNY), a tax-exempt organization affiliated with NYC Health + Hospitals, is one of the largest physician groups in New York State and directly employs nearly 4,000 physicians and allied health professionals who serve seven of New York City’s public hospitals, numerous public health community-based practices, and the city’s Correctional Health Service. SUMMARY Under the general direction of the program director or designee, the Habilitation Assistant supports individuals in achieving greater independence and personal growth through meaningful daily activities and individualized program plans. This role is responsible for assisting with personal care, implementing skill development activities, ensuring safety and well-being, and promoting inclusion both within the program and in the community. The Habilitation Assistant helps maintain a supportive, engaging, and homelike environment that fosters dignity, learning, and participation for all individuals served. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Habilitation Assistant will include, but are not limited to the following: • Provide direct support and personal care to individuals, assisting with daily living activities such as toileting, feeding, dressing, and hygiene as needed., • Support implementation of individualized program plans and behavior support strategies under the guidance of a Habilitation Specialist., • Encourage and assist individuals in developing self-help, communication, and social skills that promote independence and community participation., • Supervise and engage individuals in classroom activities, community outings, recreational programs, and skill-building opportunities., • Maintain close supervision of individuals during all program activities and community trips to ensure safety and positive engagement., • Drive agency vehicles to transport individuals to and from program sites, appointments, community activities, and other outings as needed., • Collect data and document progress on program goals, behavior plans, and treatment objectives as directed., • Prepare concise written documentation, including progress notes, incident reports, and communication logs., • Participate in interdisciplinary team meetings, case conferences, and in-service training to enhance skills and contribute to program planning., • Assist in maintaining a clean, organized, and safe program environment., • Participate in recreational activities, including swimming or gym sessions, providing direct support as required., • Dispense prescribed medications after completing all required training and certification as an Approved Medication Administration Personnel (AMAP), under the supervision of a registered nurse., • Respond appropriately to crisis situations and medical emergencies, including administering first aid and contacting emergency personnel when necessary., • Ensure actions and interactions promote respect, dignity, and the prevention of abuse or neglect., • Substitute in other program areas as needed to ensure continuity of care and coverage., • Perform other related duties as assigned by the Program Director or designee.
