About the Role We are seeking a Driver with a clean driving record and a strong commitment to supporting older adults with their transportation needs. The Driver will safely transport participants to and from center-based programs, trips, and special events, while ensuring their comfort and well-being. This role plays a vital part in helping otherwise homebound individuals engage in educational and recreational activities that support connection, independence, and healthy aging. Responsibilities Safely transport Older Adult Services participants to designated destinations. Drive participants to trips and special events as needed. Maintain accurate daily transportation route sheets. Ensure participant safety at all times, including: Assisting participants in and out of vehicles Enforcing seatbelt use Securing wheelchairs properly Collect and record transportation contributions when applicable. Assist with maintaining and submitting transportation data. Complete required reports (e.g., incident reports) and submit to appropriate departments. Maintain vehicle cleanliness and complete daily G.O.A.L. sheets; coordinate routine maintenance with the Transportation Coordinator. Serve as a substitute for other QCH drivers when needed. Perform other duties and special projects as assigned by the Transportation Coordinator. Qualifications Valid NY State Driver’s License with an excellent driving record (must be maintained). Strong interpersonal skills and the ability to engage with older adults respectfully and patiently. Ability to complete written reports and maintain accurate records. Self-motivated and resourceful; able to work independently and as part of a team. Ability to maintain confidentiality and use discretion appropriately. Responsibilities The Clinical Pharmacy Specialist (CPS) is a credentialed member of the Bassett Medical Staff who provides excellence in the continuum of care as a member of the patient care team, and facilitates a culture that engages employees, fosters innovation, and sustains program implementation. The CPS is responsible for assuring that optimal clinical services as offered under the auspices of the Section of Clinical Pharmacology, Department of Pharmaceutical Care Services are provided to patients on a daily basis. To achieve this, the CPS: Facilitates the development, implementation, and maintenance of systems/programs that promote desirable patient outcomes through the use of appropriate, safe, and cost-effective therapies, as well as promoting education and understanding of the same throughout the healthcare system. Serves as a clinical liaison and primary source of medication information for Network stakeholders including Departments of Pharmacy, Medicine, and Nursing Recommends appropriate, safe, and effective therapies including pharmacological for patients Performs wide range of clinical services including in- or outpatient services, critical care, antimicrobial stewardship, anticoagulation management services, pain management recommendations Develops or contributes to the development of various policies, protocols and service implementations Provides clinical support to other pharmacists and pharmacy unit activities Acts as an active member of various committees, subcommittees, and workgroups as appointed.
