Correctional Health Services The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Responsibilities Include (depending on location): • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction, • Ensure that medication renewals are completed in a timely manner, • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards, • In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program, • Conduct regular and multi-disciplinary case conferences, • Report on an as-needed basis to the CHS Morbidity and Mortality review committee., • Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education, • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary, • Lead at least monthly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief, Clinical Supervisor and Senior Psychiatrist (if applicable)., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints, • At least 75% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees. For smaller facilities or programs with less supervisory need, more direct patient care will be expected, • On-call responsibilities for two weeks at a time, one to times/year, for off-hours issues that require high level clinical intervention 1.Master's degree from an accredited college or university in Healthcare Administration, Health Care Planning, Business Administration, Public Administration or a related discipline; and seven (7) years of progressively responsible experience in health and medical service administration, public administration, personnel and labor relations, finance or appropriate functional discipline with an emphasis on planning, liaison and inter-organizational relationships, or related administrative or managerial functions; or 2.Bachelor’s degree from an accredited college or university in disciplines, as listed in “1” above; and eight(8)years of progressively responsible experience in areas, as listed in “1”; or 3.Satisfactory equivalent combination of education, training and/or experience. Job Description Status: Full-time, Monday – Friday (hours may vary) FLSA Classification: Non-Exempt Job Location: Jamaica, Queens NY Hourly Range: $27.47 - $32.96 Objective We are seeking a Housing Specialist for Lantern’s general population shelter. The Housing Specialist will be responsible for helping clients find permanent housing and become self-sufficient. Services will include assisting with orienting clients to the housing search process and training clients in how to conduct an apartment search, how to interview for an apartment, and how to complete housing applications. Additionally, the Housing Specialist will provide escorts to housing-related appointments and facilitate moving on by supporting clients with direct assistance during the final moving out phase. ESSENTIAL FUNCTIONS • Establish linkages and maintain solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords as a resource for clients., • Escort clients to various housing related appointments including but not limited to interviews, paperwork signings, move outs, and evaluations., • Assess client’s readiness and barriers to obtaining affordable permanent housing., • Provide supportive social services including interview prep, workshops, education, referrals, and escorting to prepare clients for successfully securing and maintaining permanent housing., • Prepare and assist clients with the housing application/interview process (i.e. HHA, CAPS)., • Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing., • Track, monitor, and report housing placement rates for all clients., • Attend housing related DHS meetings., • Monitor and assess client progression/regression in behavior and/or appearance., • Manage crisis intervention services and address all emergency situations as appropriate., • Attend interdisciplinary team meetings and case conferences as needed., • Participates in professional development activities such as in-service trainings, seminars, webinars, and conferences as needed., • A work schedule that includes evening hours. Assist clients in moving into permanent housing by providing support such as attending lease signings, assisting with applications for one-shot deals, providing moving assistance, etc., • Provides additional information and instruction on the skills required to secure and maintain permanent housing, such as how to create and maintain a household budget., • Participate in housing meetings, case conferences, staff meetings, and other meetings/trainings as assigned., • Documents all counseling, escorting, referral and discharge planning activities in client charts., • Provide comprehensive case management services to an assigned caseload., • Provide referrals for in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate., • Performs other duties as assigned. The Housing Specialist is responsible for working with clients to identify suitable and permanent housing options. DUTIES & RESPONSIBILITIES: • Interfaces with the landlord and management companies to establish a working relationship, • Create and maintain a liaison with the DHS Housing Placement Unit, • Oversees landlord and management company compliance in meeting the established housing laws, building codes, and lease agreements, • Works in conjunction with Case Management to identify potential clients who are housing-ready, • Resolves crises or conflicts with landlords and management relationships, • Ensures uniformity in maintaining fair housing standards in securing apartments, • Develops and maintains a database of available housing options, • Works towards identifying new housing opportunities for clients through ongoing networks, • Prepares and/or coordinates reports and related information related to housing codes and compliance, • Informs Case Management staff of unit readiness and/or problems with the client maintaining units, • Prepare housing packages (HRAs 2010E Application, General Population Housing Packages, HUD VASH Application Process, HPD Section 8 Process, LINC IV, IV++, and V) and collaborate with Case Management staff in preparing psychosocial applications, assisting with access to any necessary documents. The CMS will supervise staff, develop training plans, ensure completion of all required Department for the Aging (DFTA) and agency-wide trainings, and advocate for clients while maintaining high-quality case management standards. Key Responsibilities * Provide ongoing supervision, guidance, and support to Case Managers, including staff evaluations and professional development. * Screen inquiries to determine eligibility for case management; provide information and referrals when appropriate. * Complete intakes within 3 business days and submit for assignment. * Ensure all required forms and documentation are completed accurately. * Transfer referrals via the Stars Rating System (STARS) for meal authorization and verify receipt with Home Delivered Meals (HMDL). * Advocate and intervene on behalf of clients with other agencies. * Accompany Case Managers on home visits as needed. * Perform other duties as assigned.
