The Primary Care Physician (PCP) in our organization demonstrates: • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient., • The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient., • It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not)., • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs., • The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office., • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market., • PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. Remote Shifts: Monday - Friday 4pm - 12am Monday - Friday 8am - 4pm Friday, Saturday, Sunday: 8am - 9pm Essential Functions: • Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities., • Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage., • Complete expected number of UM activities per day/shift as specified under team specified under team specific responsibilities in established timeframes., • Authorize treatment when Medical Necessity Criteria is met., • Consult with Physician Advisor, Psychologist Advisor and/or Clinical Leadership as clinically appropriate., • Generate denial letters based on physician reviews and in accordance with state regulations as needed., • Work directly with members, natural supports and providers in the community during at least 6 days per month., • Facilitate care coordination with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community based services., • Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment at admission, during treatment, and at discharge., • Monitor member’s length of treatment and work collaboratively with providers and other systems to address barriers to member receiving treatment in a less restrictive environment., • Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management., • Document provider quality concerns in accordance with CBH policy/procedure., • Conduct provider and/or stakeholder meetings at least quarterly., • Use data to inform clinical decision-making., • Maintain documentation in CBH’s electronic system consistent with agency and industry standards., • Respond to care coordination correspondence within 24-48 hours or sooner if necessary., • Work collaboratively with team members including assisting with the training of newly hired employees as requested, covering work when peers are out of the office as assigned and finding coverage when absent., • Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultations., • Ensure highest quality of customer service when interacting with members, providers, and other stakeholders., • Participate in grievance/complaint hearings as assigned., • Display commitment to member and family driven care and principles of resiliency and recovery., • Participate in NIAC preparation and site visits as needed., • Maintain an accurate daily log of work completed., • Utilize identified resources to determine eligibility and third-party liability., • Review and share treatment history and pertinent clinical information about members receiving a crisis evaluation with provider to help inform recommendations., • Complete an average of 20 - 40 UM activities per shift., • Conduct electronic submission & telephonic pre certifications daily to determine appropriate level of care for members in crisis., • Track information, such as bed availability to assist with care coordination as needed. The Municipal Court Pretrial Unit (MCPT) focuses on reducing mass incarceration by supporting clients before trial through a wide array of rehabilitative initiatives and programs to divert clients from incarceration. A law clerk assigned to the MCPT Unit is a key member of the defense team who provides legal representation to indigent individuals accused of crimes in Philadelphia. Law clerks represent Defender clients in various courtrooms and stages of their cases. Law clerks practice in Preliminary Arraignment Court, Bench Warrant Court, Discovery Court, Common Pleas Arraignment and more. Law clerks conduct comprehensive confidential legal interviews at the local prisons, essential to effective client representation, write motions, file investigation requests, social service referrals and more. Law clerks appear before Bail Commissioners and makes arguments related to pre-trial supervision and detention. They also argue appeals to bail decisions before Municipal Court Judges. Law clerks also conduct confidential pre-arraignment socio-bio interviews to gather mitigation to inform bail decisions. What You’ll Do: • Arraignment Preparation: Prepare arraignment court sheets, gathering all relevant client documents, and ensure all information is accurate for proper transfer to client files., • Case Review & Advocacy: Review clients’ charges and records, advocating for the least restrictive means of pretrial supervision before the Bail Commissioner., • Client Communication: Communicate with clients’ family members both by phone and in person, providing updates and responding to inquiries., • Courtroom Rotation: Rotate weekly through various courtrooms, and interact with Judges, Court Staff, Prison Staff, Magistrates and/or Commissioners to gather relevant information for clients' cases., • Client Interviews: Conduct in-depth confidential initial legal client interviews, gathering relevant information regarding clients’ cases, charges, and personal circumstances.
