The Primary Care Physician, Partner (PCPP) in our organization demonstrates: • Accountability for outcomes: The PCPP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their risk adjusted panel of at least 400 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors., • Coaching for health: The PCPP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients., • Simplifying for action: The PCPP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals. We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCPPs become and remain successful in partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCPP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP’s are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership. The PCPP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCPP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCPP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • The PCPP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient., • The PCPP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient., • It is expected that the PCPP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not)., • The PCPP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs., • The PCPP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office., • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCPPs will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market., • PCPP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the market., • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with, an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company., • Performs other duties as assigned and modified at manager’s discretion. It is expected that PCPP will make themselves as available to their patients as possible by being open and available for walk-in visits and answering phone calls and messages in a timely manner., • Service Orientation — PCPPs provide care that they would want for a family member or for themselves to each patient at every interaction., • Evidence Based Medicine — The PCPP remains updated on evidence-based medicine, but also recognizes that factors outside of traditional medicine, like lifestyle and nutrition, have a large impact on patient health outcomes. The PCPP stays up to date on clinical, nutritional, and lifestyle based interventions to improve outcomes., • Physician Leadership is integral to good healthcare, so the PCPP must be willing to continuously work to develop and improve leadership skills for the benefit of one’s patients, their team, their center and the company., • Quality — Our patients deserve the highest quality of care. This requires a willingness to work with the care teams towards achieving high quality outcomes and quality measures. At the same time, PCPPs will always be looking for ways to continuously and systematically improve their practice of medicine and the operations of their center., • Influence — PCPPs must competently and compassionately influence their patients, their teams, and themselves to achieve the best outcomes., • Self-Care — A PCPP can take the best care for their patients when they are adequately caring for themselves. That means physically, mentally, socially and spiritually. Physician wellness is important for sustainability and promoting the health of physicians, staff, and patients., • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company, • Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes., • Fluency in English, verbal and written. There may be jobs in some centers that require fluency in other languages, and this will be made known at the time of application. MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required * Current, active MD licensure in State of employment is required * A minimum of 6 years clinical experience in geriatric, adult or family practice setting, including 2 years of ChenMed PCP, Senior Partner experience required * Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred, Board Eligibility is required * Once Board certified will maintain board certification in their terminal specialty by doing necessary MOC, CME and/or retaking board exams as required * Must have a current DEA number for schedule II-V controlled substances * Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment. Essential Duties and Responsibilities: • Handles telephone, written and in-person inquiries in a courteous, professional, thorough, and timely manner to assure member satisfaction., • Assists callers on inquiries regarding coverage decisions, grievances, appeals, benefit interpretation, eligibility verification, medical group/PCP, explanation of how the plan works and how to utilize services, assists with information regarding referrals or authorizations, member complaints, identification card requests., • Must Report to the Doral HQ., • Responsible for maintaining customer service levels (abandonment rate, speed of answer and service level) within established parameters., • Is knowledgeable on plans benefit, CMS and Plans policies and procedure., • Applies high level of subject matter knowledge to solve a variety of common business issues., • Provides quality services effectively to increase the overall level of customer service and satisfaction as measured by CMS., • Identifies the differences between a Grievance, Appeal or Coverage Decision and process the request through the appropriate channels., • Identify, handle, document and/or route Utilization Management inquiries appropriately, • Identify and document Coverage Determinations., • Documents all inquiries and complaints., • Interacts with other departments about member issues or concerns., • Approaches work in a meticulous and thorough manner, pays attention to detail and follows company scripts., • Manages time effectively and prioritizes tasks to meet deadlines., • Assists customers, family members and others with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner., • Complies with company policies and procedures and maintains confidentiality of customer medical records in accordance with state and federal laws., • Ensures compliance with all HIPAA, OSHA, and other federal, state and local regulations., • Participates in meetings, trainings and in-service education, as required., • Highly regulated environment., • Time frames to handle issues are in accordance with CMS time frames., • Minimum of High School diploma or equivalent., • Minimum of 1year healthcare experience is required, with a preference for plan experience., • Working knowledge of the Privacy and Security Health Insurance Portability and Accountability Act (HIPAA) regulations., • Excellent communication skills, attention to detail, ability to set priorities appropriately and meet strict deadlines and the ability to manage multiple tasks simultaneously is required., • Excellent listening, interpersonal, verbal and written communication skills with individuals at all levels of the organization., • Excellent computer knowledge is required, including proficient knowledge of Microsoft Office, • Familiarity with healthcare laws, regulations and standards., • Ability to compute basic arithmetic and work with numbers., • Must be patient in dealing with an elderly population and sympathetic to hearing or vision deficiencies., • Ability to work effectively independently, work with very little supervision and in a team environment., • Ability to read, analyze, and interpret technical procedures or governmental regulations., • Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public., • Ability to define problems, collect data, establish facts, and draw valid conclusions., • Strong decision-making, analytical skills., • Must be self-motivated, organized and have excellent prioritization skills., • Must be able to work well under stressful conditions., • Must be able to work in a fast paced environment., • Fluency in English and Spanish. Job Description Position Purpose: The Population Health Medical Assistant conducts the coordination of care to improve Hemoglobin A1c, Preventive Oral Health Visits and Pap Smears for patients within disease specific populations. He or she is responsible for the development, implementation and alignment of CHI goals and the existing quality improvement efforts and support the achievement of PCMH recognition by using data to better manage our patient population. POSITION REQUIREMENTS / QUALIFICATIONS: Education/Experience: Experience working with culturally diverse clients. Working knowledge regarding Joint Commission, NCQA, HEDIS, UDS requirements. An ability to learn and understand those standards and elements. Prior experience working with patients with chronic conditions preferred. Licensure / Certification: Certified as a Medical Assistant in the State of Florida. Maintain current CPR certification from the American Heart Association. Must have a valid Florida Driver’s License. Skills / Ability: Excellent communication and interpersonal skills. Sensitivity in working with diverse cultural groups. Demonstrate excellence in both internal and external customer service, adhere to national patient safety goals as defined by the Joint Commission. POSITION RESPONSIBILITIES (NON-EXEMPT POSITION):  Attends orientation sessions, scheduled meetings, and face-to-face consultation with supervisor.  Consistently gives attention to detail.  Assist the Population Health department in orienting and educating patients and their families on the importance of routine diagnostic testing in addition to screening tests and visits such as Hemoglobin A1c, Preventive Oral Health Visits and Pap Smears.  Assist in coordination and care requirements for patients within the disease specific populations.  Responsible for documentation of and scheduling of tests and diagnostic services and interacting with members of the care team to ensure that tests, procedures, and consults are completed.  Responsible for maintaining a database to track patients when they obtain services in an emergency department, hospital, or other healthcare facility.  Under the direction of the clinicians, they communicate care plans to patients and their families.  Coordinates home visits if warranted for non-compliant patients.  Assist in providing relevant self-management support for patients with chronic illnesses identified by the clinical team.  Assist families to access the health care system and assign to a medical home.  Assist with data collection and generation of patient registry reports.  Communicate effectively and expresses ideas clearly; actively listens and always follows appropriate channels of communication.  Work with patients both in person and over the phone to remind and review their plan of care and progress towards achieving their goals.  Maintains flexibility to access families during non-traditional hours.  Collects and report results of outcomes and referrals, i.e., A1c, Pap Smears and Preventive Oral Health Visits, etc.  Collects specific data as required to meet the goals of the program.  Maintains accurate records of the activities performed.  Follow-up and track missed appointments.  Track coordination of referrals for outcome results.  Provides documentation of any patient contact when abnormal results are followed-up.  Attends scheduled in-services and trainings.  Works to achieve patient outcome goals as established in the Section 330 Health Care Plan.  Promptly informs supervisor of problems encountered in the line of duty.  Follows CHI Policies and Procedures.  Maintains 100% of monthly productivity goals.  Reports to work on time and ready to work with minimal absenteeism.  Assist Population Health Department with meeting deliverables for clinical intervention grants and goals.  Adheres to Confidentiality Policies and Procedures / HIPAA Regulations.  Treats patients with respect and dignity.  Participates in patient/community education (internal and external) and home visits when necessary.  Performs other duties as assigned.
