General Description Responsible for providing supportive counseling and independent living skills training to assigned residents and assisting the Program Supervisor in implementing agency policies and procedures. Duties and Responsibilities • Assists the Program Supervisor with managing the day-to-day operations of the program., • Coordinates the daily activities of the program., • Meet with assigned residents at least once a week., • Inclusively develops, implements, and modifies person-centered goals with recipients of the program to foster quality of care., • Participates with the residents from diverse backgrounds in the development, evaluation and modification of individual service plans and reviews. Monitors plan objectives on a regular basis, • Responsible for completing all required documentation in a timely manner including but not limited to writing progress notes on a daily or weekly basis and document services provided with appropriate notes., • Completes chart work, reports and/or reviews/audits department charts as needed., • Trains and assists residents in the activities of daily living including household maintenance, food purchasing, meal planning and personal care activities., • Trains and assists residents in self-administration skills of medication as needed. (Responsible for learning side effects of medication.), • Trains, orients and assist staff in their day-to-day job responsibilities., • Coordinates 24-hour staff coverage of program., • Coordinates admission of residents into congregate program., • Responsible for maintaining Petty Cash, revolving loan, van petty funds and monitors house- related bills., • Assists residents in establishing vocational /educational goals., • Assists residents in improving socialization skills., • Participates with residents in recreational activities., • Provides crisis management, case management services, and behavioral interventions. (Includes making collateral contacts.), • Coordinates or attends treatment team or discharge meetings, • Notifies the appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency., • Distributes residents’ Money Management and Personal Need Allowance monies as indicated. Assist residents in completing and/or submitting financial records., • May participate in the Utilization Review Committee., • Conducts scheduled house meetings with residents, • Provides transportation as needed, for shopping, medical appointments etc., • Conducts weekly house inspections to determine what repairs are needed. Reports problems to Program Supervisor to submit a work order., • Screens calls from apartment residents, assesses situation and, if necessary, contacts the on- call person for further direction., • Implement fire safety program, including conducting fire drills at community residence as needed., • Relays residents and/or program specific information to appropriate agency personnel., • May be required to provide coverage at other residential sites as needed., • Attend administrative meetings in Supervisor’s absence., • In the absence of the supervisor, responsible for the overall operations of the congregate program in conjunction with working with the Program Director., • Meet at least once a week with the Supervisor., • Attends work related workshops/conferences as program schedule permits., • Learns and adheres to all agency policies and procedures on an ongoing basis., • Be proactive in the development of new ideas, forms, and program procedures., • Accepts and follows through on all other duties and responsibilities as assigned. Qualification and Competencies • Minimum High School Diploma with (5) years’ experience in a human service-related field. Bachelor’s degree or higher preferred., • Experience with mental health population or substance use disorder/treatment experience preferred, • Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families, and the community., • Critical thinking and sound judgement skills are necessary. Physical Demand and Working Conditions • Close in-person interaction with clients in a group-home setting, staff, and/or outside representatives, • Transport clients in personal or agency vehicle as needed, • Provide coverage at all residential sites as needed to ensure staffing coverage, • Participates in on-call rotation and responsibilities outside of normal schedule, including weekends and/or holidays and available during emergency situations., • Ability to work amongst diverse social, cultural and economic groups., • Required to take all mandatory training courses Vision: To promote health, wellness, and independence by providing access to housing opportunities and services through the expansion of programs and partnerships.
