The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Senior Psychiatrist is the psychiatric leader of the facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic and evidence-based therapeutic interventions are provided for the individuals incarcerated in the New York City jail system. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for elderly and medically complex patients with comorbid mental illness., • Directly treat and oversee the psychiatric care of additional patients housed in NIC/WF/CDU, • Direct patients to the appropriate level of mental health care, within the facility or in the hospital as needed., • Close communication with psychiatric and medical providers at Bellevue Hospital to ensure continuity of care as patients transition between the two facilities., • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief and/or Senior Clinical Supervisor, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. If medication is not in Pyxis, technician can have it added to Pyxis., • When outside methadone clinics call to verify last dose and date of methadone, technician will review Accumed methadone administration records to ascertain the patient’s last dose and date of methadone administration., • If there are requests for a patient profile, the individual should be instructed to get the profile themselves from report manager. Requests from other agencies should be referred to the pharmacist on duty., • Prints and reviews all transfer reports, checks if the patients is on medication, prints patient medication profile and fills needed doses of medication for subsequent checking by pharmacist. If patient is on methadone or DOT medications, tells pharmacist of such., • Transports patient cassettes to medication clinic areas and exchanges same when necessary., • Files completed fill lists, updates lists, distribution lists, methadone sheets and other materials according to procedure., • Checks, rotates and restocks all drug stock areas. Fills Pyxis machines as required. Files Pyxis reports with non pyxis stock records and retains them for 1 week. On a monthly basis checks for outdated medications in Pyxis, clinic floor stock, pharmacy stock and other areas where pharmacy stores medications. For non pyxis floor stock, maintains established par levels based on individual clinic or infirmary need. Department Overview The Residential Crisis Support Program Respite Center (CRC) is a Community Access Health Division program. The CRC is an alternative to hospitalization program offering short-term respite care in a friendly and supportive home-like environment. Guests are provided with the opportunity to learn and use recovery and relapse prevention skills to ameliorate their current psychiatric crisis and to provide more effective tools to cope with future crises and avoid hospital-based care. Recovery-oriented services are provided 24-hour and day and include peer support, self-advocacy education and self-help training. The Residential Crisis Support Program offers people experiencing a mental health or emotional crisis an up to 28 days residential alternative while supporting individuals in continuing their established daily activities such as visiting with friends, attending a doctor’s appointment or going to work. Position Overview The Per Diem Respite Worker is responsible for all tasks related to the Respite daily operation during shift hours including providing services and support to CRC guests. Some responsibilities of the Respite Worker are to develop partnerships with guests experiencing crisis utilizing trauma-informed, recovery-oriented intentional peer support and promotes the provision of a comfortable, secure, and trusting environment where guests are listened to and respected by staff. Respite Workers are skilled with the mental health system with a focus on guest’s personal recovery process. This helps to create a sense of community through the mutual sharing of lived experiences and offering encouragement and support to participate in daily activities within the RCS community. Job Description The Mental Health Association is seeking Per Diem Crisis Outreach Supervisors for our new program, 988 Mobile Crisis Outreach Team, operating out of the Hudson County location. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shifts are on an as-needed basis for this position. Specifically seeking holiday and weekend coverage. The Surveyor will serve as a team member or team leader on various types of surveys (i.e. re-certification, comparative, complaint investigation, and revisits) for long-term care and non-long-term care surveys, which can include; ambulatory surgical centers (ASC), Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICFs/IID), end-stage renal disease (ESRD), Psychiatric Residential Treatment Facility (PRTF), hospital, critical access hospital, and hospice facilities. Surveyors travel to healthcare facilities nationally conducting surveys to assess compliance with requirements and regulations guiding the quality of care for residents/customers of the facilities. The responsibility of the Surveyor is to apply approved survey protocols for conducting on-site surveys. Specific activities include the following: • Conduct on-site surveys of Medicare-Medicaid certified health care facilities to determine compliance with Federal regulations., • Surveys require preparations such as off-site preparation, tour of the facility, observation of care, record review, interviews of staff and families, etc. Determine if care, treatment, and services are consistent with Federal requirements., • Evaluate many aspects of the healthcare facility system from the quality of care to the consumers’ rights and physical plant., • Communicate the non-compliance issues with key personnel throughout the site review and during the exit conference to assure an understanding of the deficiencies cited., • Determine if a plan of correction is acceptable., • Conduct follow-up visits and monitoring surveys to ascertain if facilities’ plans of correction to resolve the deficient practice have been implemented., • Training: Assist with facility and bureau training. Summary The Clinical Rapid Response Nurse (CRRN) is a key member of the Crisis Response Team, responsible for providing immediate, comprehensive, and ongoing medical and behavioral health support to our high acuity clients, characterized as having chronic medical conditions, severe substance use disorder, high impairment, and severe mental health conditions. The CRRN will triage and intervene based on a tiered risk system (High, Moderate, Low) and collaborate with interdisciplinary teams to prevent hospitalizations, enhance medical and psychiatric stability, and improve quality of life for individuals at Urban Pathways. The RN will be reporting to the Program Coordinator while receiving clinical supervision from our agency’s Medical Director. Key Responsibilities: 1. Clinical Rapid Response & Triage: • Conduct proactive and emergency assessments for high acuity clients, • Utilize the risk tier system to prioritize care interventions:, • High-Risk: Requires twice a week monitoring and immediate interventions., • Moderate-Risk: Requires weekly follow-ups and early intervention strategies., • Low-Risk: Requires regular follow ups, preventive care, and maintenance., • Enhance program team’s collaboration with emergency services, hospital systems, and psychiatric crisis teams to ensure continuity of care., • Assist program staff in Initiating and coordinating preventive interventions to reduce ER visits and hospitalizations. 2. Medical & Psychiatric Care Coordination: • Help program staff monitor and manage chronic medical conditions (e.g., diabetes, hypertension, COPD, cardiovascular diseases)., • Assist program staff in assessing and supporting substance use treatment, including medication-assisted treatment (MAT) and harm reduction strategies., • Assist program staff with medication reconciliation and adherence, ensuring proper psychiatric and medical treatment adherence., • Assist the Total Wellness with providing health education to residents and staff on chronic disease management, medication safety, and harm reduction. 3. Crisis Intervention & Behavioral Health Support: • Implement interventions designed to prevent behavioral and psychiatric crises, • Assist, when applicable, with de-escalating behavioral and psychiatric crises using trauma-informed and evidence-based approaches., • Enhance program team’s efforts in collaborating with psychiatric providers and social workers to ensure appropriate interventions for our high acuity clients, • Assist program staff in conducting suicide risk assessments and engage in safety planning as needed., • Support detoxification and withdrawal monitoring, referring to higher levels of care when required. 4. Interdisciplinary Collaboration & Documentation: • Work closely with social workers, case managers, peer specialists, and housing staff to ensure coordinated care., • Maintain accurate, timely, and detailed clinical documentation in compliance with regulatory and Urban Pathways’ Clinical Response Rapid Team Standards. This includes use of our electronic health record and documentation being completed with 48 hours. (Same-day entries required in cases involving tier 1 high acuity clients). Documentation also consists of conducting screening and assessment tools., • Participate in team case conferences to help develop and refine individualized care plans., • Enhance program staff liaison between external healthcare providers and Urban Pathways staff to ensure seamless transitions of care and continuity of care for high acuity clients across programs. 5. Training & Staff Support: • Provide training and education to Urban Pathways staff on medical, psychiatric, and substance use issues, including risk identification, emergency response protocols, and health management., • Mentor and support junior nursing and medical staff and other clinical team members, fostering a collaborative and trauma-informed care environment., • Act as a resource for other team members by offering guidance on complex cases involving high-acuity clients with multifaceted needs. 6. Job Description The Mental Health Association is seeking a Crisis Outreach Supervisor for our new program, 988 Mobile Crisis Outreach Team, operating out of the Hudson County location. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shift times available: 11PM - 7AM Qualified applicants for the position of Crisis Outreach Supervisor will have, at minimum: • A Master’s Degree in a mental health related field from an accredited institution, a professional license (LSW, LAC) and two years of post-master’s full time equivalent professional experience in a psychiatric setting required., • A professional with experience in a crisis setting is preferred., • A valid NJ driver’s license and a good driving record., • The ability to provide effective crisis response and de-escalation as related to community mental health in the community.
