CAMBA’s Cure Violence Program works with 16 – 24 year old at risk youths who are or may have potentially been involved in street violence, community residents, businesses and community-based organizations to implement Cure Violence: The Campaign to Stop the Shooting. Cure Violence is a public health strategy to reduce and prevent shootings and killings. This program will focus on a defined area identified as a “hot spot” in Brownsville, Brooklyn for shooting violence within the 73rd Precinct. The program works to prevent gun violence by: 1) mediating conflicts that may end in gun violence, 2) changing the thinking of individuals who are most at risk of being victims or perpetrators of violence, and 3) working closely with a broad coalition of local partners to promote a visible community-wide message that shooting is unacceptable behavior. Position: Outreach Worker Reports To: Assistant Program Manager Location: 444 Thomas Boyland Street, Brooklyn, NY 11212 What The Outreach Worker Does: • Reach out and market the program to the community via street outreach in order to recruit clients., • Assist in organizing responses to all shootings that occur in the targeted catchment area within 72 hours of receipt of notice of the shooting or killing., • Prepare marketing materials and market the program to community., • Attend and help facilitate social/peer support events (i.e., community events and shooting response meetings, etc)., • Review all documentation establishing clients' eligibility for program and make file copies., • Assist clients in completing all CAMBA intake applications and forms., • Create and maintain client files., • Conduct initial intake or assessment of clients and clients' families and/or periodic reassessments., • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization., • Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade clients to participate more fully in this process., • Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, etc., • Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes., • Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area)., • Provide all required information for weekly/monthly/quarterly/annual reports., • May conduct client’s and/or families’ periodic re-assessments., • May schedule appointments for client with referral organizations., • May escort clients to appointments (educational, medical, social service, etc.)., • Equally at home conversing with gang members, community activists, law enforcement personnel, and public officials., • Subject to a criminal background check and random drug testing., • No pending criminal cases or prior convictions for sexual assault, child abuse or domestic violence or history thereof., • Strong knowledge of the Brownsville community., • Familiarity with Brownsville 73rd Police Precinct community challenges of teens and young adults there; strongly preferred., • Ability to work a flexible schedule including morning, evening and weekends as needed., • Must have understanding of, sensitivity and commitment to working with families of youth that have experienced gun related trauma., • Case Managers should be prepared for various outdoor weather conditions during their regular workday. Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Job DescriptionDescription: Title: Intensive High-Risk DV Case Manager Department: Prevention & Intervention Reports To: Director, Domestic Violence & Trauma Services Location: 126 Stuyvesant Place (SI Family Justice Center) Position Status: Full Time FLSA Status: Non-Exempt Pay & Benefits: The pay range for this role is generally $46,300 - $51,300 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Domestic Violence & Trauma Services Program offers a comprehensive suite of services to survivors of domestic violence with enhanced focus on education and linkage to health & wellness through both individual and group counseling supportive services. CHASI works in collaboration with the Staten Island Family Justice Center to expand reach and capacity. Domestic violence and trauma services are co-located. Services are provided at two different locations, 26 Bay Street Site and the Staten Island Family Justice Center (FJC). The Intensive Case Manager will manage a high-risk caseload with a specific focus on working with complex, intersectional cases of domestic and gender-based violence including, but not limited to, family violence, elder abuse, human trafficking, and sexual violence. The Intensive Case Manager will navigate clients through the many complex systems called into action in service plans, such as behavioral health services, medical services, criminal justice, family court, and child protective services. The Intensive Case Manager will be a strong advocate who can provide ongoing support to clients with the police, prosecutors, housing, shelters, schools, public assistance, and other service providers. The Intensive Case Manager will provide on-going and enhanced follow-up with clients to ensure that clients are accessing and successfully connecting with the array of supportive services available at the FJC and through off site partnerships and linkages, creating positive relationships with on and off-site partners. DUTIES & RESPONSIBILITIES: • Conduct initial comprehensive assessments, safety planning, and risk assessments with clients., • Navigate the client to execute their service plans, including making active referrals, accompany clients to various appointments, provide coaching to clients, deliver weekly or monthly education encounters, plan logistics for plan adherence reminders, conduct regular follow-up, and assist with transportation to get to and from appointments. Document all case management activities in accordance with guidelines and standard procedures., • On a weekly basis, review and update the program client list, deliverable tracker, and internal database., • Participate in internal and external meetings and trainings as assigned., • Align all activities with CHASI’s mission, core values, and culture., • Other duties as assigned. Requirements: QUALIFICATIONS: • Education: Bachelor’s degree in health, human services, education, social work, or mental health with 1 year of qualifying experience OR Associates degree in related field with minimum 3-4 years of qualifying experience; qualifying experience includes work with individuals at high risk of contracting HIV/AIDS, history of mental illness or substance use, harm reduction, domestic violence, immigrants, LGBTQ, etc., • At least 3 years of experience working with culturally diverse and vulnerable populations who have experienced trauma. Must be able to demonstrate the ability to engage and serve the domestic violence population targeted by this program and have strong engagement and follow-up experience., • Fluency in Spanish highly preferred., • Have familiarity with orders of protection and navigation of the criminal and family court systems., • Must have basic knowledge regarding HIV, HCV, STI, PEP/PrEP and Harm Reduction methods., • Excellent MS Office (Outlook, Word, Excel, PowerPoint, Teams) and internet navigation skills., • Ability to work independently and as a team; be team player with an ability to work with diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates others to perform at their best., • Must possess the ability to interact with all levels of external and internal partners with a high level of professionalism and tact in representing CHASI., • Strong verbal and written communication skills, including capability to listen and follow instructions; able to give, provide direction and follow up on task; high attention to detail and accuracy.
