Job Description IRON Recover and Wellness Center Position: Case Manager Location: Trenton, NJ Type: Full Time Days: Monday - Friday Salary: $52,000 About the Role: The Case Manager provides comprehensive case management and support services to clients participating in substance abuse treatment programs. This position helps individuals access needed medical, psychological, social, educational, and vocational services to promote recovery and long-term stability. The Case Manager works collaboratively with treatment teams, community partners, and families to ensure coordinated and effective care. Role Responsibilities: * Provide advocacy and support to help clients navigate complex systems (legal, social services, healthcare, etc.). * Support youth-focused services, school partnerships, and family engagement, with strong connections in Region 7. * Facilitate or co-facilitate recovery-oriented groups and psychoeducation sessions as needed. * Maintain regular contact with clients through in-person meetings, phone calls, or telehealth sessions. * Collaborate and communicate effectively with treatment team members and community partners. * Assist clients in achieving life goals such as independent living, community involvement, employment, building natural supports, and engaging in meaningful leisure activities. * Advocate for clients in obtaining needed services, public benefits, and addressing concerns related to unmet needs. * Provide on-call crisis intervention services as scheduled. * Participate in treatment team meetings for clients on your caseload and individuals requesting support services. * Support individuals in building connections that promote recovery and long-term stability. * Complete daily assignments and tasks delegated by the Team Leader, including coordination with counselors, medical staff, probation/parole officers, and external providers. * Maintain communication with family members or client-identified support networks when appropriate and with client consent. * Perform other duties as assigned, verbally or in writing. Additional Responsibilities: * Maintain accurate, timely, and confidential client records in accordance with federal, state, and agency guidelines (including HIPAA and 42 CFR Part 2). * Complete all required documentation, including progress notes, reports, and client file updates. * Track and report client outcomes, program metrics, and other assigned performance measures. * Ensure full compliance with agency policies, funding requirements, and ethical standards. * Encourage staff and clients while demonstrating initiative and effective problem-solving skills. * Assist in data collection, quality improvement activities, and analysis for program evaluation. * Maintain confidentiality, professional boundaries, and ethical conduct in alignment with agency policies and New Jersey professional regulations. * Ensure all client records are maintained according to agency standards, policies, and procedures. * Engage in ongoing professional development, including seminars, continuing education, and training in addiction and mental health services. * Complete all required annual trainings. * Serve as a liaison with external agencies, partners, and community resources as needed. * Participate in the development and improvement of program services and quality initiatives. * Perform other duties as assigned. Qualifications: * Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a related field preferred. * Minimum of 1–2 years of experience in case management, counseling, or human services preferred. * Experience working with individuals with substance use disorders, co-occurring mental health conditions, or in recovery-oriented settings preferred. Skills: * Ability to work effectively with diverse populations. * Ability to appropriately route client concerns and problems to the proper resources or professionals. * Ability to maintain professional boundaries and adhere to the defined role and responsibilities of the position. * Strong critical thinking and problem-solving skills. * Ability to utilize the Cyber system and other electronic documentation tools. * Strong organizational and time-management skills. * Excellent verbal and written communication skills. * Ability to understand and apply techniques, procedures, and changes to respond to varying client needs and situations. * Ability to communicate effectively with supervisors and directors in order to enhance skills, receive feedback, and adapt to agency culture and policies. Additional Requirements: The candidate must pass a Physical & Drug Screening (Including THC) & possess the ability to attend and successfully complete all required trainings. Individual must meet physical requirements of the job to include sitting at a meeting table or desk, reaching, lifting, bending, standing, stooping, twisting, climbing steps, and driving a vehicle. The candidate must pass required Criminal Record History Clearance, FBI and Child Abuse History Clearance. Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.
