Job Description RiseWell Community Services is seeking a Part Time Assistant Case Manager who will report to the Site Supervisor within the Outreach Services Department at Pilgrim. In this role, you will assist Case Managers by providing selected services to ICM clients. Our Outreach programs provide an array of services designed to engage individuals who are reluctant to seek help or experience barriers to access. Services are brought to the person where they are located, whether that be at home, the street, a hospital, or another location. As a part of this team, you’ll assist those we serve to have their basic needs met by providing referrals, linkage, and hands-on assistance with obtaining food, employment, housing, and financial assistance. The Family Recovery Center provides individual and group therapy to children, families, and adults directly impacted by alcohol and drugs.
