Job Description SUMMARY Paragon Professional Services, LLC is currently seeking a qualified Lead Armed Transportation Officer for DHS/ICE in Newark, NJ. Wage/Salary Range: $41.00 / hour Applicants will be contacted via phone or email within ten (10) business days of submittal. Provide trip documentation to the Transportation Coordinator in a timely manner. · Notify Operations Supervisor, Transportation Coordinator and/or Project Manager of any route variations/changes. · Ensure that any required detainee documentation is obtained from the transferring facility/agency prior to departure. · Ensure that the assigned vehicle maintenance and service records are in corresponding binders. · Report any safety related transportation issues immediately and complete a vehicle repair order and give to the Supervisor in charge of Fleet Management. · Conduct pre-trip vehicle inspections on your assigned vehicle prior to departing the HUB. Complete post vehicle checks at the completion of your assignment. · Ensure all required equipment is present in your assigned vehicle prior to departing. This shall include, but not limited to, first aid kits, insurance card, flares, security devices, etc. (See equipment list assigned to each vehicle.) · Report for work at your scheduled time (assigned by Operations Supervisor daily). · If assigned to a stationary guard post, such as hospital duty, ensure compliance with Post Orders and hospital protocol. · Ensure you always have in your possession; guard card, weapons permit, ICE ID, Company ID, medical card (if applicable). · Forward any gas receipts to the Supervisor in charge of Fleet Management. · Report any accidents, incidents and/or injuries associated with your transportation duties. We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. CAMBA operates 6 single adult shelters and 6 family shelters and one drop in center. Each shelter offers clients a supportive, structured therapeutic, safe, and drug-free facility. Homeless (often mentally-ill and substance-abusing) men and women are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing. PROGRAM: CAMBA GARDENS is a 209 unit supportive/affordable housing development located in the East Flatbush section of Brooklyn. CAMBA Housing Ventures (CHV), the housing development subsidiary of CAMBA, developed these units. 146 of these units are set aside for formerly homeless individuals and families. 117 units are for Population I individuals, chronically homeless single adults who suffer from Serious and Persistent Mental Illness or who have a co-occurring Mental Illness and Chemical Addiction (MICA). 15 units are for Population III individuals, chronically homeless single adults who have a substance abuse disorder that is the barrier to independent living and who also have a disabling clinical condition. 14 units are for Population V chronically homeless families, in which the head of household suffers from a substance abuse disorder, a disabling medical condition or HIV/AIDS. Program Description Health Home Plus (HH+) is an intensive Health Home Care Management (HHCM) service established for defined populations with Serious Mental Illness (SMI) who are enrolled in a Health Home (HH) serving adults. Position: Project Coordinator Reports To: Program Manager Location: 2244 Church Avenue, Brooklyn What The Project Coordinator Does: The person filling this position is responsible for: aiding qualified individuals who require social service assistance. This includes interviewing and evaluating clients, formulating service plans and goals, in order to aid clients in meeting their Mental Health and Medical needs. This should be done through the use of appropriate community resources/referrals. Field work is mandatory, each client must be seen a minimum of 2 times per month or as needed, specific to client need. Project Coordinators are expected to provide intensive case management services to select populations (as defined by the New York State Department of Health) who also have a Serious Mental Illness (SMI) and are enrolled in a Health Home (HH) serving adults. Case Management services are provided for a period of up to 1 year, possibly longer, dependent on client need. • Carry and maintain a caseload of up to 20 clients, providing assessments, developing care plans, provide service referral/navigation, and crisis intervention, as needed., • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information., • Conduct required field work. All clients must be seen, in-person, a minimum of two times per month. The Van Dyke facility will provide 30 units for New York/New York III formerly homeless families. The remaining 71 units will provide housing for low-income households, including a live-in superintendent. Fifty-two units will be set aside for NYCHA residents. CAMBA will serve 30 chronically homeless families, or families at serious risk of becoming chronically homeless, in which the head of household suffers from a serious and persistent mental illness or is diagnosed as mentally ill and chemically addicted under Population Option II. Position: Part Time Security Guard Reports To: Assistant Program Manager Location: 603 Mother Gaston Blvd., Brooklyn, NY 11212 What The Part Time Security Guard Does: • Monitor and ensure the security and fire safety of the entire facility., • Employ crisis prevention/intervention and crisis management skills when conflict occurs., • Patrol buildings and grounds of facilities where CAMBA programs are located., • Examine doors, windows, gates and all fire exits to determine that they are secure and in accordance with all fire regulations., • Observe and report irregularities such as unusual tenant behavior, trespassers, fire hazards, leaking water pipes, and security doors left unlocked., • Monitor tenants and facility via security camera systems., • Conduct fire safety patrol and report findings or fire safety concerns to appropriate supervisory personnel., • Conduct security screenings of all tenants and visitors entering the facility., • Issue commendations to support positive tenant behaviors such as peers helping peers, compliance with rules and regulations, etc., • Counsel tenants that violate rules (such as loitering, smoking, or carrying forbidden articles) and complete infraction reports., • Observe departing employees, visitors and/or tenants to guard against theft of CAMBA property., • Document in the security log book and incident report forms property damage, unusual occurrences, and incidents with employees, visitors and tenants., • Maintain assigned equipment per shift., • Sign equipment in and out for each shift., • NYS Security Guard license, • Sufficient Education to comprehend basic reading, writing, and math to execute the required functions of the role., • Must have an 8-hour Pre-Assignment security guard training course certificate., • Must obtain a 16-hour On-the-Job security guard training course certificate within 90 days of employment., • Must complete the 8-Hour Annual In-Service Course each calendar year., • Must have and maintain a NYS Security Guard license., • Must obtain NYS Security Guard registration and must maintain registration during employment., • Must obtain Fire Guard certification and maintain it throughout employment.
