We are currently seeking a Director of Social Services to oversee and ensure that the medically related emotional and social needs of the residents are met on an individual basis. Responsibilities: • Oversee all aspectes related to the social services department to ensure all the emotional and social needs to the residents/patients are met., • Work with the IDT team to develop and implement policies and procedures for the identification of medically related social and emotional needs of the patient/resident., • Participate in planning, development and implementation of care plans, resident assessments, and discharge planning., • Interview patient/resident/families as necessary and in a private setting. Involve the patient/resident/family in planning objectives and goals for the patient/resident.
