Job Description The ICL PROS (Personalized Recovery Oriented Services) Program offers rehabilitation, counseling, therapeutic activities, and peer support each day to individuals living with severe mental illness. The Associate program director is a professional and administrative position with responsibilities for planning, developing, evaluating, and administering services as defined by the NYS Office of Mental Health for a PROS program. Job Description About the Role The Unaccompanied Alien Children Program provides temporary residential care and other supportive services to Unaccompanied Alien Children (UAC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, our program provides a safe and nurturing environment for 33 youth in a Basic Shelter program located at our licensed residential facility in Yonkers, NY as well as a Transitional Foster Care program for 40 youth in certified foster homes in the Bronx. In both programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions as well as link them with family members and/or other sponsors in the United States, so they are able to move forward with their lives and thrive. Job Description Under the general supervision of the Program Director or Designee, functions as part of a team in a community residence providing client-centered, strength-based rehabilitation and recovery services to assigned consumers that relate to the care, nurturing, treatment, education, socialization, habilitation, recovery/healing and support of adults diagnosed with a mental illness and in some instances, co-occurring substance abuse disorder. These tasks focus on supporting, instructing and assisting recipients of mental health services to develop the skills needed to attain personal goals, live, be educated, work, and socialize successfully in the community environments of their choice. ESSENTIAL TASKS: • Secures information, such as medical, psychological, and social factors contributing to the individual's situation, and evaluates these and the individual's capabilities. Based on this information, completes and maintains consumer treatment plans to include quarterly service-plan reviews, assessments, medical records, and changes in treatment and/or medications either manually or electronically as instructed., • Provides supportive counseling in 1:1 or group formats that assist the consumer to modify attitudes and behaviors as needed., • Maintains up-to-date, accurate individual case records on each assigned consumer and develops measurable and objective service plans that maximize consumer rehabilitative abilities in accordance with regulatory guidelines. Job DescriptionSummary:The Safe Horizon DV Shelter Program provides safe and confidential spaces for DV Survivors and their children. We are committed to providing these services with empathy, compassion, professionalism, non-judgement, and respect. Our spaces respect survivors’ right to self-determination and focus on giving families and individuals the space, both emotional and physical, to heal from the victimization and trauma they have experienced. The Housing Specialist position provides assistance, information, and support to residents in domestic violence shelters around securing safe, appropriate housing using subsidies and/or other available resources in the city that suit the individual’s needs and to maintain residential stability. Working in conjunction with the case management staff, under the supervision of the Supervising Social Worker, the Housing Specialist is responsible for developing the best possible safe housing plan together with each resident in the program. The housing specialist must also be aware of and understand current housing subsidies, public benefit programs, and develop resources to include establishing relationships with landlords and brokers. Job Description Client Care Coordinator Supervisor PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. If needed, this individual will ensure that recommendations of achievable goals be implemented on each family’s service plan, leading to permanent housing, and strengthening the overall permanency outcome for clients in shelter. MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • Knowledge in three or more of the following areas:, • Experience in supervision of social workers, • Experience in client assessment, service plan development, and case management, • Child and adolescent development, • Parent-child relationships, family dynamics, and diagnostic classifications, • Client Care Coordinators to develop a team of skilled professional staff, including recruitment, training, supervision, evaluation, and staff scheduling to achieve the objectives of both CRF and Agency Funders, • Guide client care coordinators in the development, implementation, and achievement of program goals, • Handle administrative, operational, and programmatic issues that might affect client care coordinator staff, • Assign cases to client care coordinators for newly admitted clients to be assessed immediately upon admission and managed throughout shelter stay, • Ensure the timely completion of comprehensive bio psychosocial assessments for each client to understand strengths and service needs, • Work effectively as a part of a multi-disciplinary team to enhance client engagement and the delivery of quality service, • Enhance the delivery and coordination of mental health and related services to clients in shelter, • Ensure that Client Care Coordinator reports are clearly written and capture assessment findings and recommendations, • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being, • Ensure that Client Care Coordinators are providing short-term counseling, outreach services, crisis intervention, risk assessment, safety planning, and pyscho-education for clients, • Collaborate with ACS and/or prevention services agencies, and participate in conferences to advocate on behalf of clients as needed, • Ensure that Client Care Coordinators are providing clients with appropriate referrals and linkages between mental health providers, government agencies, and other related community-based services as needed, • Serve as agency/program liaison between DHS, government agencies, community-based organizations and/or groups, • Develop and Implement group work to be facilitated by Client Care Coordinators to address family issues, enhance life skills etc., • Ensure that Client Care Coordinators are completing all documentation in a timely manner, • Ensure that Client Care Coordinators assess each client/family for supportive housing, complete supportive housing applications when applicable, and accompany residents to supportive housing interviews when needed, • Ensure that Client Care Coordinators complete and discuss exit summaries with each client, and provide clients/families with referrals and supportive services identified in their new community, • Participate in family meetings with case work and housing teams to discuss planning efforts and progress, • Networking and establishing successful linkages with government and community-based organizations to create resources for both families and staff, • Work in collaboration with the Director of Social Services and the Housing Coordinator to ensure that all services lead to permanent housing placement, • Complete special projects and assignments as needed or identified by leadership, • Must work one late a week from 11 am -7 pm to accommodate both staff and families, • Provide on-call crisis intervention support to Client Care Coordinators, • Perform other duties as assigned CLIENT Job Description Title: NSC Program Manager Location: Bronx, NY FLSA Status: Full Time (35 hours) Exempt Role Summary: The Neighborhood Safety Council (NSC) Program Manager will coordinate and facilitate a community-based safety council in the 44th Precinct as part of NYC's Gun Violence Prevention Task Force (GVPTF). This position requires a dynamic, community-oriented professional who can build relationships across diverse stakeholder groups, facilitate collaborative planning processes, and coordinate implementation of neighborhood safety initiatives.
