As part of the Comprehensive Medication Management Program, conduct interviews, develop, facilitate and communicate a plan of care to remove barriers for members who have been identified as having difficulty with medication adherence. Continuously evaluate plan of care and revise as needed. As part of the Medication Therapy Management Program, conduct and document according to CMS standards annual comprehensive medication reviews. POSITION OVERVIEW The Medical Director engages in the professional, organizational, and administrative activities of the provide quality care to clients based upon their medical and psycho-social needs. The Medical Director develops, implements, evaluates the medical practice delivery model in accordance with specific goals. The responsibilities of the Medical Director are to be performed on a full time basis and are designed to provide medical leadership, services, direction and coordination in the provision of medical care and physician services. Job Description POSITION SUMMARY Under the direction of the Program Manager, the Case Manager is accountable for providing intensive case management and crisis intervention services for clients and their families and other individuals identified as their support system.
