The Wound Care Preventionist is responsible for conducting regular, proactive skin and wound assessments to prevent the development of pressure injuries and other wound-related complications. This clinical role supports overall patient safety and outcomes by identifying early risk factors and ensuring proper preventative measures are implemented in coordination with the clinical team. What Youll Do • Perform scheduled skin integrity and wound prevention assessments on at-risk patients/residents, • Conduct thorough assessments to identify early signs of skin breakdown and collaborate with nursing staff to initiate appropriate interventions, • Develop and implement individualized, evidence-based treatment plans to support wound prevention., • Partner with facility healthcare teams to coordinate care plans and treatment updates., • Document all prevention visits, observations, and recommendations accurately in the electronic medical record (EMR), • Monitor the effectiveness of preventive measures and report any changes or concerns to the wound care nurse or provider, • Ensure consistent use of appropriate support surfaces, positioning techniques, and barrier products as needed, • Track and follow up on high-risk individuals to ensure continuity of preventive care, • Communicate clearly and professionally with clinical staff to reinforce skin integrity protocols, • Maintain compliance with facility policies, clinical standards, and regulatory guidelines, • Ensure continuity of care through consistent communication and collaboration., • Multi-Facility Travel & Scheduling visiting multiple facilities weekly., • Ensure timely and efficient care delivery with professionalism and compassion. Position Details: Infinite Medical P.C. is seeking a highly motivated and passionate Physician with Board Certification in Psychiatry to collaborate with and oversee Nurse Practitioners and/or Physician Assistants providing psychiatric services in Long Term Care facilities. This physician will ensure the highest standards of behavioral health, psychiatric, and psychotropic medication management, while supporting regulatory compliance and clinical excellence across the organization. Responsibilities: • Clinical oversight and collaboration with Psychiatric Nurse Practitioners and/or Physician Assistants, • Twice Monthly meetings/check-ins with assigned psychiatric practitioners, • Phone supervision of assigned Nurse Practitioners/Physician Assistants delivering psychiatric care, • Provide consultation to providers via electronic communication (phone, email, telehealth, etc.), • Review psychiatric evaluations, treatment plans, progress notes, and medication management documentation, • Support appropriate diagnosis and treatment of common behavioral health conditions, including but not limited to schizophrenia, bipolar, schizoaffective and other serious mental illnesses, depression, anxiety, dementia-related behavioral disturbances, psychosis, and mood disorders, • Guide best practices related to psychotropic medication use, monitoring, and deprescribing when appropriate, • Collaborate with interdisciplinary care teams, including primary care providers, nursing staff, and facility leadership, • Execute and ensure compliance with State-to-State collaboration and/or supervision requirements and applicable scope-of-practice regulations, • Medical Degree (MD/DO), • Board Certification in Psychiatry (preferred) or completion of a ACGME or AOA psychiatric residency program in the United States, • Multi-state licensure and/or IMLCC participation a plus, • Clinical experience in psychiatry, psychology, or behavioral health, • Experience working with or collaborating alongside Advanced Practice Providers, • Strong leadership, communication, and clinical judgment skills, • Commitment to quality improvement and evidence-based psychiatric care, • Experience in a long-term care, skilled nursing, or geriatric setting (preferred), • Comfort with remote collaboration and electronic medical record review Infinite Medical P.C. is an equal-opportunity employer.
