Job Description Summary: Streetwork Project is the oldest street outreach program for homeless and street-involved youth in New York City. The program has grown from its beginnings in 1984 as a storefront project to encompass both outreach and comprehensive harm reduction services. The core service is long-term, one-on-one counseling, which is offered in a non-judgmental and holistic manner utilizing a harm reduction approach that focuses on building trust, fostering self-esteem, exploring options and choices, and empowering youth to change their high-risk behaviors. The Public Benefits Coordinator will work with clients on accessing emergency and long-term benefits. The core of the benefit assistance will include advocacy for acquiring and navigating cash assistance, food stamp (SNAP), Medicaid and disability-income programs (SSI and WeCARE). Job Description POSITION SUMMARY CPF is seeking to hire experienced players, coaches or teachers to teach free beginner tennis lessons to children and seniors in NYC parks. Lessons for seniors are typically held in the Spring and Fall on weekday mornings. The Care Manager develops and implements a patient-centered, comprehensive Plan of Care for the member, in partnership with his/her caregivers and providers. The Care Manager also is a central point of contact for the family and child, to ensure that their needs are being met, their voices are heard, and their needs and preferences are integrated into plans of care. Likewise, the Care Manager is the key link among all service providers for each family, working to ensure that all providers are operating with complete, current and consistent information, and that they are aligned in the goals they each are pursuing with the family and child. Job Description POSITION SUMMARY City Parks Foundation Learning Gardens brings year-round, field and school-based garden science learning to NYC school children and community members. Our gardens are outdoor classrooms that provide vibrant green space in some of New York City’s most densely populated neighborhoods. We provide programming in various community gardens in four boroughs. Using our garden partner sites, CPF presents engaging interdisciplinary lessons to expand student learning around urban ecology, horticulture, soil science, and sustainable urban food systems. Students in 1st – 8th grade and their teachers receive resource materials, curricula, and tools to enhance their understanding of ecosystem services and address the unique challenges of growing food in an urban environment. Casual Youth Advocates work part-time and on-call, filling in for full-time youth advocate absences at the Overnight and Outreach program. The Alumni Advocate will serve as a pivotal staff member within the organization by providing support, guidance, and resources to our alumni pool for two years post-graduation from exalts core program. The Alumni Advocate is responsible for ensuring that exalt alumni continue to make gains in the organizations three key focus areas of academic advancement, employability, and criminal justice avoidance. Coordinate tutoring services with external partners, including transcript collection, contractadministration, data, and ongoing communication. Support leadership in evaluating tutoring and academic outcomes annually. Employability and Professional Development Support alumni during graduate internships and transitions to employment. Assist alumni in securing external internship placements and employment-related resources. Plan, schedule, and lead professional development workshops, alumni events, corporatepartnerships, and youth advocacy initiatives. External Engagement Cultivate and maintain partnerships with post-secondary institutions, vocational schools, andworkforce development organizations. Data Management and Reporting Maintain timely and accurate data through daily and weekly updates across all program areas.
