Our client is a community and faith-based organization dedicated to supporting justice-involved youth through mentorship, advocacy, and structured programming. They are seeking a Youth/Mentor Advocate to serve as a Credible Messenger within a juvenile justice program supporting youth ages 13–18. This role will lead mentoring sessions, support youth development, and collaborate with families, justice partners, and community organizations to help participants build positive life pathways. Job Description POSITION SUMMARY: The Violence Intervention Case Manager at Chester Community Coalition (CCC) provides blended case management services to gunshot victims within the Chester, PA community who are referred from local hospital systems. Case management services are provided to community members impacted by intentional violence as they heal from injuries post-discharge from the hospital setting. Case managers are also essential to establishing relationships and partnerships with local treatment organizations that may serve those impacted by gun violence in Delaware County. Job Description The Family Support Partner (FSP) is a parent/caregiver of a child with emotional or behavioral challenges who has successfully navigated the system. This lived experience allows the FSP to have a unique ability to engage with families receiving services and support them with empathy and understanding. The FSP will work within the Clinical Management Department. The FSP will primarily provide support and guidance to caregivers of youth referred, actively receiving treatment and/or discharged from any child acute and/or residential treatment program. Job Description Philadelphia’s Office of Safe Neighborhoods (OSN) prioritizes public safety within our communities, focusing on ending gun violence in our city. Our objective is to reduce and end gun violence through a comprehensive strategy that tackles the root causes and offers social services to Philadelphians in need. OSN’s social support initiatives prioritize mental health services, such as the Community Crisis Intervention Program (CCIP), through community outreach engagement. The Community Crisis Intervention Program (“CCIP”) uses credible messengers to serve as Community Crisis Outreach Workers to prevent and reduce gun violence in Philadelphia. CCIP employs proactive strategies to foster meaningful relationships in Philadelphia’s most violent neighborhoods, steering those involved in violence (or at a high risk of violence) into alternative positive choices, mediating neighborhood conflicts, and responding to neighborhood crises related to violence. OSN contracts three organizations to employ credible messengers, including the Philadelphia Anti-Drug/Anti-Violence Network (PAAN), the Institute for the Development of African American Youth (IDAAY), and Eddie’s House. The Philadelphia Anti-Drug/Anti-Violence Network (PAAN) is a non-profit agency dedicated to addressing drug abuse and violence within communities across the city. The CCIP Data Manager will be responsible for auditing, cleaning, and analyzing data collected during CCIP outreach shifts and will work with the Upper Management Team at PAAN to maximize the efficient utilization of PAAN’s resources. Regular feedback and standardized reports from the Data Manager will help guide when and where teams should be deployed and/or fortified and ensure there are no gaps in services needed or areas covered. Job Description POSITION: Program Coordinator/Mentor (SEASONAL) REPORTS TO: Program Manager POSITION SUMMARY: This position is part of a seasonal team responsible for the Urban Affairs Coalition's Summer Youth Employment and Spark Youth Engagement (SYE) program. The Summer Youth Employment program provides paid summer internships and support to youth ages 14-24 through the city of Philadelphia’s summer youth employment program. The six-week program runs from late June through mid-August and includes career exposure, on-the-job mentoring and professional development. The Spark Youth Engagement Program offers career exposure and career awareness activities for in school youth and is offered year-round. Initiatives under Spark Youth Engagement include career exposure, professional development, and mentoring with the goal of entering the workforce. The Program Coordinator/Mentor will provide direct support to the Program Manager in all aspects of the program from recruiting and enrolling placement sites to supporting the enrollment process for youth. This position requires extensive field time and travel in the Greater Philadelphia area. The Program Coordinator/Mentor will be expected to provide their own transportation via public transportation or personal vehicle. The Program Coordinator/Mentor will oversee youth participant activities to ensure program placement sites are programmatically and contractually compliant with program guidelines. All documentation and communication will be directed to the assigned DHS worker or court personnel via e-mail correspondence, telephone calls, and with hard copies to be kept in client’s file., • Case Manager will facilitate the family’s transportation for appointments with other service providers by providing directions, tokens, as needed, and accompanying on public transportation or driving them to appointments and meetings in their own vehicles., • Case Manager must attend monthly team sessions to determine and monitor service appropriateness for each youth and to inform the team of the client’s adherence to orders of the Court and to the program., • Case Manager must attend all mandatory IPS trainings and activities hosted by the Department of Human Services or designee., • Participates in meetings to ensure priorities are clear, coordination is good and communications are open., • Cooperates and communicates as a multi-discipline staff team member through formal meetings, informal discussion and other participation as necessary., • Models traits of a responsible team member, executing job responsibilities, open communication, good follow-through; supports team members to do the same. This may involve accompanying clients to appointments at Public Assistance and Social Security offices., • Provide 24-hour crisis intervention on a rotating basis. This involves coordinating care with mobile crisis, the Philadelphia Children’s Crisis Response Center (PCCRC), or psychiatric hospital. This may involve accompanying the youth and caregiver to the PCCRC or hospital., • Ensure continuity of care when youth are moving from one level of care to the next to prevent gaps in service coverage. This is especially critical for youth moving from out of home placement (e.g., foster care, group home, RTF, AIP) back home., • Actively participate in all staff meetings and provide consultation and support to team members as needed., • Coordinate and lead interagency meetings for assigned cases for the purpose of service coordination and treatment and aftercare planning., • Maintain required records and documentation in compliance with Agency, county, state, and Managed Health Care regulations., • Maintains collaborative working relationships with service providers., • Participate in professional development as assigned, including mandatory annual trainings and 10 hours of elective trainings per year as recommended by BCM supervisor., • Function in compliance with agency policies/procedures and federal, state and local law., • Participate in Continuous Quality Improvement (CQI) activities as requested by BCM Supervisor or Compliance Officer., • Other duties as assigned.
