As a Senior CaseManager (with clinical specialism), you'll coordinate rehabilitation, bring professionals together around shared goals and help clients achieve greater independence through expert, person-centred support. This is an opportunity to manage a varied caseload of adults and children with complex neurological conditions while using your clinical expertise to deliver meaningful outcomes. You'll work autonomously, collaborate with multidisciplinary teams and play an active role in developing services, mentoring colleagues and maintaining the highest professional standards. If your background is in Occupational Therapy or Physiotherapy, there is also the opportunity to maintain a mixed caseload combining therapy and case management. What's in it for you Car allowance Home or office-based working with flexible full-time or part-time hours Opportunity to maintain a mixed therapy and case management caseload (where clinically appropriate) Ongoing clinical supervision, peer support and professional development Opportunity to influence service development and mentor less experienced colleagues Your responsibilities as Senior Case Manager Complete comprehensive assessments and develop goal-focused rehabilitation plans tailored to each client Coordinate multidisciplinary teams, external professionals and support services to deliver effective rehabilitation programmes Recruit, supervise and support care teams while identifying training and development needs Produce high-quality clinical documentation, reports and case management records in line with professional standards Monitor rehabilitation progress, risk, budgets and outcomes while advocating for clients throughout their recovery journey Contribute to clinical governance, business development, research, audits and service improvement initiatives What we're looking for in a Senior Case Manager HCPC registration and a recognised Occupational Therapy or Physiotherapy qualification with a neurological specialism Proven background managing complex neurological rehabilitation caseloads within case management Strong knowledge of clinical governance, rehabilitation legislation and multidisciplinary working Ability to produce professional clinical reports and manage complex rehabilitation programmes Full UK driving licence with willingness to travel and participate in an on-call rota Working hours: Full-time or part-time, with participation in a shared daytime weekend on-call rota.
