Job Description The Mental Health Association is seeking Peer Support Specialist for our new program, 988 Mobile Crisis Outreach Team, operating out of Hudson county. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Job Description Job Title: Bilingual Clinical Case Coordinator Job Summary: CASES is currently seeking a highly organized and detail-oriented Bilingual Clinical Case Coordinator for our Pretrial Services Case Management Team. Rapidly after release into the program, the Bilingual Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Bilingual Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. The Bilingual Clinical Case Coordinator will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. The goal is to increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. Program participants include youth and young adults, people with substance use disorders and mental illnesses, and individuals who are homeless. SRP participants have not been found guilty of a crime and are not mandated to engage in treatment or other services. The Case Manager will be expected to assist this population in stabilizing their lives, maintaining their housing stability, and attaining their physical, emotional, vocational, educational, and social goals. The ideal candidate will have solid clinical knowledge as well as experience working with individuals and or families that have been impacted by mental illness, substance abuse, legal, criminal justice, and medical issues. The Behavioral Health Center offers a comprehensive suite of services for mental health including First Episode Psychosis program, medical services, opioid treatment, substance use disorders services, a specialized unit for people with Intellectually Disability, an inpatient Extended care unit focusing on the needs of our homeless patients and prevent recidivism, ketamine assisted therapy, and inpatient and outpatient services for adults and children. RESIDENT SUPPORT TASKS: • Resident Integration – show empathy to residents upon entry; participate in orientation of new residents, explaining the types of clinical services, recreational activities, and ICL programs that are available; instruct the resident in daily living skills, socialization skill enhancement, and conflict resolution, • Service Plan Support – obtain and develop resource info for residents in all aspects of their care; disseminate this information to residents in a manner that is clear and understandable; advocate on behalf of residents with outside service providers and within ICL; collaborate with residents to utilize assessment info to complete and maintain resident service plans, offering an Integrated Healthcare approach to services and ensuring that plans include consideration of the residents’ cultural and ethnic background, customs, needs, beliefs and preferred language; use cognitive-behavioral techniques (e.g. role-plays, skills training, plans of action) to assist the resident in meeting service plan goals; conduct weekly meetings with residents to follow-up on service plan goals and objectives; effectively and efficiently write, create, understand and follow verbal and written instructions and questions from residents, • Community Support – actively look for ways to assist residents with compassion and hope; believe and help residents work toward recovery; refer residents to community resources and other orgs.; provide support & advice to significant others of the residents and Transitional Veterans Residence staff; provide motivational interviewing to residents as needed; accompany individual during the first-day transition from Transitional Veterans Residence to meetings with community providers, and in establishing connections with local community resources, • Travel & Recreational Planning – provide residents on both an individual or group basis with transportation for housing placement, service appointments, recreational, or other service plan-related purposes as appropriate; plans, arranges, and/or escorts residents to social or recreational service activities, • Education Support – assist residents in finding and getting into vocational training or other training opportunities; conducts in vivo assessment and living skills training as needed; serves as a role model to residents including modeling appropriate interpersonal interactions, appearance, demeanor, • Finance Support – assist residents with developing a savings and money mgmt. plan within the scope of the Dept. of Homeless Services and Veterans Administration regulations; optimize resident income through advocacy with the HRA and VA who will offer and provide residents both group and individual instruction in budgeting, banking, and resident skills such as shopping wisely from a price and product perspective, credit, taxes, etc., • Medical Support – provide counseling in 1:1 or group formats that assist residents with modifying attitudes and behaviors as needed, by increasing understanding of self and others and by addressing service needs, strengths, abilities, and environmental resources to attain goals; accompany residents on regular scheduled appointments or emergency visits to medical treatment facilities, social agencies, government offices, or other locations associated with the treatment or assistance of the resident; participate in substance abuse services with the substance abuse counselor/specialist; administer CPR or first aid to staff or residents who require such services; use sound judgment in identifying and solving problems and knowing when to seek assistance for residents, • Conflict Resolution – perform crisis assessment and verbal/non-verbal crisis intervention; mediate conflicts between residents, negotiating ground rules for relationships; may provide critical time intervention procedures; maintain self-control in crisis situations, working with residents, families, and staff in a caring and respectful manner, • Discharge Support – facilitate individualized services to residents, meeting their diverse needs and focusing on the discharge planning goal with due understanding of and consideration for cultural differences, • Perform other job-related tasks as assigned in an organized and detail-oriented manner…, • Please Note: To perform this job successfully, an individual must be able to perform each essential duty listed satisfactorily.
