Patient safety is the number one priority in the delivery of healthcare. The LPN oversees Medication Administration, monitors client health, and collaborates with program staff around client treatment. HMH is hiring for a full-time Behavioral Health Clinician responsible for direct clinical work with clients in the outpatient mental health clinic, with a priority focus on those in the CONNECT program. The BH Clinician will also be responsible for coordinating and accessing care for their clients. The Behavioral Health Center offers a comprehensive suite of services for mental health including First Episode Psychosis program, medical services, opioid treatment, substance use disorders services, a specialized unit for people with Intellectually Disability, an inpatient Extended care unit focusing on the needs of our homeless patients and prevent recidivism, ketamine assisted therapy, and inpatient and outpatient services for adults and children. Responsibilities The attending psychiatrist if the clinical lead of the assigned inpatient team and is responsible for all aspects of patient care on the team, including safety and quality. Direct Patient Care • Provide direct care for a specific number of patients on the unit, including appropriate and timely documentation., • Complete admission assessment for all patients within 24hr of inpatient admission, and re-assess patients in a timely fashion. Document re-assessments in progress notes consistent with policy., • Participate in treatment plan meetings and ensure timely documentation consistent with policy., • Participate in Behavioral Codes on the unit, providing leadership to the staff and patient community, • Participate in Medical Codes on the unit until the Code Team arrives, • Ensure appropriate use of de-escalation, including appropriately and timely use of restrictive interventions, as per policy, to ensure patient safety., • Work with all teams to monitor length of stay, particularly ensuring that, • Patients who are refusing medications are taken to court promptly, and that, • Patients who have reached their 18th day are referred for state hospitalization for continuing treatment, • Identify patients who would benefit from AOT and work with their team to ensure that the application is completed., • Work with medical-consultation team ensure that medical problems are properly managed, • Ensure that legal statuses are up to date and accurate, • Work with the UM department to conduct MD to MD reviews and participate in any appeals to ensure authorization of inpatient stay., • Assist in the preparation for surveys, • Maintain awareness of the standards for certification by the various regulatory agencies Qualifications • Must be board eligible or board certified in Psychiatry, • M.D. or D.O. from an accredited school of medicine, • Completed residency training in an accredited program within Psychiatry Annual Base Salary: $236,900* (board eligible); $247,200* (board certified) based on 40-hour work week. Job DescriptionPosition Summary Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. POSITION SUMMARY: CHASI’s Next Step Resource and Recovery Center provides opportunities for people in all stages and forms of recovery to deliver and attend individual and group support activities, initiate and conduct outreach, and events celebrating recovery and reducing stigma associated with substance use. The Center operates 24 hours a day, 7 days a week. It hosts several services, including Recovery Center peer-driven individual and group recovery services, counseling and case management, overdose prevention services, harm reduction, and recovery-oriented community outreach, education, and the CHASI Peer Training Institute. The Peer Training Institute is designed to promote professional growth and development for people in recovery from substance use and support the institutions that hire them. Currently, the Peer Training Institute includes training opportunities in the areas of Peer Recovery Coaching (CRPA - Certified Recovery Peer Advocate), Administration, and Janitorial. This position also includes supervision of at least one Training Specialist. The Training Institute Coordinator will serve as the lead trainer and coordinator of the CHASI Peer Training Institute and the lead organizer and point of contact for the newly established CHASI Alumni Association. DUTIES & RESPONSIBILITIES: • Serve as training lead for the CHASI Peer Institute roster of trainings, including the CCAR Recovery Coach Academy, Peer Ethics, and Medication-Supported Recovery., • Support curriculum development and evaluation of training modules related to harm reduction, trauma informed care, substance use, recovery support, thriving in the workplace, and skills for working with clients., • Schedule and coordinate training offerings from CHASI, as well as those that may be offered by partner organizations., • Provide direct supervision and development to training staff., • Register students through required systems (FOR-NYC and Protraxx)., • Co-facilitate informational sessions for students throughout the application process., • Develop and deliver training agenda and activities., • Maintain all files and complete data entry and routine reports on a monthly, quarterly, and annual basis., • Attending training as needed to ensure competency in service delivery, both in content and skills, and maintain credentialing, to ensure the success of the Peer Training Institute.
