(aging population that wish to live in their home, NOT in a nursing home) AND TBI survivors. Salary commensurate with experience. Bachelors AND 3+ years experience required Looking for a Case Management Supervisor to work a part time caseload (see Service Coordination) job duties below. The supervisor is expected to meet any waiver participants prior to the completion of the Revised Service Plan (RSP) developed by a Service Coordinator under their supervision. Any visits that exceed 10 in a month will be assisted by the TLC Directors. The Supervisor will have supervisory meetings with staff on at least a monthly basis, and review and sign-off on all service plans and monthly notes. In addition, they will monitor the quarterly tracking spreadsheet to ensure proper time management by staff. A supervisor MAY maintain an active caseload of waiver participants up to 25. However, this caseload must be reduced from the maximum limit allowed in relation to his/her supervisory responsibilities. The Service Coordinator Supervisor must complete a monthly face to face visit with each TBI waiver participant on their caseload. They must meet with the participant in their home at least quarterly. Job Description In this Supervisor role you will assist IPRO’s Senior Director and the New York State Department of Health (NYSDOH), Office of Aging and Long-Term Care, Long Term care, with oversight of surveillance and investigation activities related to nursing homes (long term care). These activities are mandated to evaluate facility performance and monitor the quality of care and services provided. Duties include but are not limited to ensuring the quantity and quality of IPRO staff work output with acceptable outcomes; working with IPRO’s designated staff to ensure correct invoicing are submitted for staff; coordinating and assisting in providing preceptor surveillance training to IPRO surveyors; participating as a member of an interdisciplinary survey team and producing written documentation and draft Statements of Deficiencies (SOD); attend meetings and other duties assigned. The position will be located at the NYS DOH lower Manhattan OR Central Islip office. Non-travel days are based at the designated office or telecommuting, if approved. QUALIFICATIONS: • Demonstrate strong interpersonal skills with the ability to facilitate positive customer satisfaction in situations that are adverse or in controversy., • Ability to successfully work independently with minimal supervision, as well as in a team environment., • Excellent written and verbal skills with the ability to communicate professionally with co-workers, supervisors, providers, medical and administrative personnel, and residents/patients., • Ability to translate collected information into a clear, concise, legal, and defensible document., • Excellent investigative and analytical skills., • Strong computer skills with working knowledge of Microsoft office products, and the ability to learn new programs., • If approved to work remotely, must have secure internet that meets the speed required to perform all work duties, must have a workspace that provides privacy for all work-related calls, and must be able to secure all work-related items when not in use., • Must have a valid driver’s license & the ability to travel to on-site facilities review assignments. EDUCATION & EXPERIENCE: • NYS Licensed, Registered Nurse, required., • A minimum of two (2) years of experience in performing utilization review, claims adjudication, medical review, fraud investigation, surveillance, or monitoring activities, OR, three (3) years of clinical or administrative experience, OR A bachelor’s degree in nursing and two (2) years of clinical or administrative experience and two (2) years of supervisory experience., • Surveyor Minimum Qualifications Test (SMQT), required. The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. The salary range for this position is $109,750.00- $115,996.50. Actual salary range and /or hourly rate depends on a variety of factors including experience, education, credentials, location, and budget. Assures the maximum quality of life for all residents by maintaining privacy, accommodating individual preference, promoting independence, encouraging and assisting participation in community activities and events, and communicating respectfully at all times., • Assists residents in and out of bed and chair/wheelchair with the correct use of the transfer techniques if required. Depending on residents’ weight and physical abilities, may require the assistance of another aide or the use of a mechanical lift to move the resident. If resident is unable to reposition self, provide assistance to turn and reposition resident intermittently., • Maintains current knowledge of each resident’s service plan and delivers care and services accordingly. Assists residents in all Activities of Daily living, including bathing, personal grooming, using the bathroom and perineal care, and dressing in a caring and compassionate manner. Monitors resident wellbeing and appearance continuously and assists as needed throughout the day., • Assists in maintaining cleanliness, condition, and supplies in resident suites by observing and reporting housekeeping and maintenance needs and obtaining care supplies from the community or family as needed. Folds or hangs clean clothing items and places soiled items in hamper. Removes soiled bedding and replaces with clean linens as needed. May be required to provide, remove and clean bedpans, urinals catheter bags, and bedside commodes., • Assists residents to dining area for meals or provides meals in room as required. Provides assistance as needed to encourage proper nutrition and hydration during meals and snacks. Documents as required., • Engages residents in meaningful life enrichment programs, as guided by Activity Assistants, Memory Care Supervisor, and Memory Care Director., • Assists in maintaining resident charts. Documents assistance provided to residents at the end of each shift. Communicates all changes and unusual observation concerning residents physical and psychosocial functioning., • Attends training courses and participates in other professional development activities as required., • Meets dress code standards: appearance is neat and clean. Maintains good personal hygiene. Abides by safety protocols, • Reports to work as scheduled; minimal absenteeism. Notifies Supervisor in a timely manner if unable to work a scheduled shift., • Wears identification while on duty uses computerized punch time system correctly., • Must express a sincere desire to work with individuals living with dementia, and demonstrate an understanding of the uniqueness of each resident., • Must have excellent verbal skills and communication abilities., • Ability to work effectively within a team based environment., • Strong time management and organizational skills., • Ability to effectively present information and respond to questions from managers, clients, customers, and families., • Ability to calculate figures related to amounts such as percentages and area., • Must be able to clearly and proficiently communicate with people of all ages, while maintaining an energetic and positive attitude., • One (1) year of experience working as a Certified Nurse Assistant including some medical/surgical or home care experience and awareness of the needs of aging adults, preferred., • Ability to work flexible hours including weekends and evenings., • High school diploma or equivalent is required., • Must be criminally cleared by DOJ and FBI via the Live-Scan process prior to initial presence in the facility., • Be in good health and physically and mentally capable of performing assigned tasks. Good physical health and freedom from tuberculosis hall be verified by a health screening performed by a physician prior to employment., • Have current Adult First Aid card at time of employment and maintain throughout length of employment.
