Assesses clients and families to determine need for network services.  Develops Family Service Plan with client and families.  Matches client and family needs with appropriate network service providers.  Engages in collaborative service planning with network service providers.  Prepares progress reports and communicates regularly with network service providers.  Maintains documentation of clinical records in accordance with federal, state and local regulations and organizational standards.  Participates in treatment planning case conferences as a member of the multidisciplinary team.  Performs other duties as required.
