Job Description Title: Physical Therapist - Homecare - FT (Monmouth County) Status: Full Time Location: Wall Township, NJ Salary: Starting at $90,750, depending on experience. Key Responsibilities • Conduct thorough evaluations of clients in their home environments and develop individualized therapy programs with measurable short‐ and long‐term goals., • Deliver treatments aligned with the physician's plan of care, documenting progress, reassessments, and discharge summaries in a timely and accurate manner., • Act as a liaison with physicians, managed care providers, aides, and other members of the interdisciplinary team to ensure efficient coordination of care., • Supervise a Physical Therapy Assistant (PTA): assess their ability to perform interventions, ensure required co‐signatures, and monitor ongoing performance., • Provide written home‐exercise or care programs to patients/caregivers, give education based on their needs, and foster a respectful therapeutic relationship to promote goal achievement., • Participate in discharge planning, respond promptly to voicemails/messages, update schedules in the system, and document communications across the care team., • Meet productivity standards consistently, adhere to organizational competencies and standards of behavior, and carry out additional duties or projects as assigned., • Bachelor's Degree in Physical Therapy or equivalent., • Valid Physical Therapist license in the state., • Basic Life Support (BLS) certification (Healthcare Provider level)., • Minimum of six (6) months of physical therapy experience., • Must be able to travel to 4-6 patient homes per day within Monmouth County in New Jersey, • Doctorate in Physical Therapy (DPT)., • Skilled clinical evaluation and treatment planning in a home‐care setting, • Strong verbal and written communication (patient, caregiver, and interdisciplinary team), • Effective time‐management and scheduling in a mobile/home‐based role

Job DescriptionJob Description African Caregivers LLC, an approved New Jersey DDD provider, is seeking a Board Certified Behavior Analyst (BCBA) to provide behavioral oversight and consultation for individuals receiving services with behavioral acuity needs under Embedded Behavior Supports. This is a part-time contract role (approximately 510 hours per month). Responsibilities • Provide clinical oversight of Behavior Support Plans (BSPs), • Support staff implementing Positive Behavior Supports, • Participate in interdisciplinary team meetings as needed, • Provide crisis prevention and behavioral consultation, • Ensure compliance with NJ DDD Waiver requirements (Section 17.2) Requirements • Active BCBA Certification (required), • Experience with developmental disabilities and community-based supports preferred, • Familiarity with NJ DDD Behavioral Services standards, • Ability to provide documentation of credentials Compensation • Competitive hourly or monthly consulting rate, • Flexible schedule To Apply: Please send resume and BCBA certification.
