Scans current insurance card and photo identification into system * Provides, explains and reviews for accurate completion, all Registration forms, consent forms and obtains signatures as required * Provides and explains the Authorization to Release Health Information to patient at their request and documents appropriately * Discusses balances due, including past balance, co-payments, co-insurance and deductibles, referring to Patient Accounts as necessary; takes responsibility for collecting and posting payments from patients at time of service via check, money order or credit card in compliance with our policies and procedures * Offers patient's My Chart activation in compliance with Meaningful Use guidelines * Provides After visit Summary in compliance with ACO guidelines * Follow end of day check process daily to total and balance collections * Begins the check in process in EPIC. Act on notations and completes the check in process successfully * Schedules/Cancels/Reschedules patient appointments as ordered by the physician adhering to scheduling policies and procedures * Obtains HMO insurance referrals as required for maximum reimbursement of services rendered * Notifies management or other departments appropriately using Clerical Templates for various issues/requests/reasons * Maintains supply inventories and equipment necessary for the effective performance of the job; communicates supply needs to the office supervisor in a timely manner * Maintains a neat, organized, orderly environment in the reception and waiting room areas, including physician business cards, brochures, signage, etc * Closes office as needed, ensuring all patients accounted for and discharged. The BIS role includes and is not limited to engaging and working with individuals, residential staff, guardians, and community service providers to support the harmonious operation of people supported. The BIS will recognize behavioral concerns and aid to lessen and remedy negative behaviors through the development process of various supportive measures. These measures include but are not limited to conducting assessments, implementing behavior support plans and or medication monitoring plans, providing emotional support, attendance at monthly clinical team meetings, life plan meetings and staff meetings, crisis interventions, attendance at routine and emergency psychiatric appointments and the development of plans and protocols to assist the residence in implementing successful behavioral technique. The BIS will evaluate and monitor progress using interviews, observations, and direct engagement with people supported and work in conjunction with their support team. The BIS will monitor the individuals, analyze progress, and adjust behavioral plans accordingly. The BIS will modify activities, tasks, and materials to provide an opportunity for individuals to successfully integrate into their setting. Additionally, the BIS will provide training, coaching, and role modeling to residential staff in behavioral techniques. This includes the free-standing day habilitation site, individual day habilitation cases and day habilitation without walls programs. The Day Habilitation Coordinator shall oversee the development and growth of the programs with an emphasis on quality and diversity of individualized activities and experiences and shall be responsible for the safety and wellbeing of all the people we serve while in attendance at the program or in the community. There is a direct relationship of the success and growth of the program to the variety and creativeness of the program activities developed. The Day Habilitation Coordinator will embrace the forward thinking ideologies of Adapt of the Hudson Valley?and OPWDD in the movement of individuals from more restrictive and isolative environments to those which foster free thought, individuality and independence. ALL plans developed by the Coordinator of Day Habilitation will embody these tenets. II.?ORGANIZATIONAL RELATIONSHIP The Day Habilitation Program Coordinator oversees two Assistant Coordinators, two shift leaders and 18 Direct Support Professionals (DSP). These staff work on-site and off-site in “without walls” community integrated programs.
