Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a CORE Peer Specialist. Community Oriented Recovery and Empowerment (CORE) services is a program serving individuals living with severe and persistent mental illness. This program serves Medicaid HARP enrollees and works to assist individuals in reaching their recovery goals. The CORE Services Certified Peer Specialist is responsible for delivering Peer Support as well as the other direct care services offered within the CORE program. The CORE Services Certified Peer Specialist is responsible for ensuring members receive person-centered, trauma-informed and culturally competent services with a rehabilitation and recovery focus in their homes or their community. This role is responsible for facilitating the utilization of natural resources and the enhancement of recovery-oriented principles with the intention to assist individuals with initiating recovery, maintaining health and wellness, and enhancing the quality of personal and family life for long-term recovery. Essential Duties & Responsibilities • Oversee and manage a caseload of members that are enrolled in the CORE services program, seeing them on a 1:1 basis as needed, wanted, and defined by an individualized service plan., • Help individuals work on goals such as community connectedness, development of natural resources, physical and mental health wellness, education, employment, self-help, and independent living skills., • Provide services in a person-centered, goal-oriented, trauma informed and culturally competent approach which includes advocacy, outreach and engagement, self-help tools, recovery supports, transitional supports, pre-crisis and crisis supports., • Assist in selecting and utilizing self-directed and evidence-based recovery tools, including but not limited to Wellness and Recovery Plans (WRAP) or Relapse Prevention Planning, with members to develop skills for long-term recovery., • Provide health coaching and recovery support, especially on issues of self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities., • Assist with and coordinate transportation to appointments, including provision of travel training as needed., • Accompany members to behavioral health, medical and/or other appointments to support and facilitate engagement and follow through as needed., • Coordinate and communicate care planning with other providers of services., • Attend and participate in collaborative CORE team meetings., • Act as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services., • Maintain compliance with all New York State Office of Mental Health policies and procedures., • Maintain organized and accurate documents and client records using AWARDS - electronic health record (EHR) database., • Perform related duties as assigned by supervisor., • Produce comprehensive and descriptive progress notes with time, location, intervention and follow-up needed., • Document every service with strict deadlines, • Complete required extensive trainings and pass post-training test in the areas of psychosocial rehabilitation, trauma-informed care, person-centered care, motivational interviewing, substance use disorders, and mental healthRequirements: Education & Experience • High School Diploma or GED required; Bachelor’s degree preferred., • NYS Peer Certification (Provisional or Full Certification), OR NY Certified Peer Recovery Advocate (NYCRPA) Certification as well as maintaining this certification which is necessary to maintain active job status, • Lived experience required: personal experience living with a disability, history of mental health challenge and/or substance abuse and willingness to draw upon this experience to serve as a role model to support the recovery goals of individuals served, • Working knowledge of computer software and electronic health record systems, • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards, • Knowledge of Medicaid, Social Security, and other entitlement systems, • Excellent interpersonal skills required, • Ability and willingness to regularly travel using public transportation throughout the boroughs and have flexibility in hourly/weekly schedule, • Excellent verbal and written communication skills, including ability to effectively communicate with internal and external colleagues, • Excellent computer proficiency (MS Office – Word, Excel, and Outlook), • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service, • Ability to work independently and to conduct assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices

Job Description Incident Coordinator Eihab Human Services Title: Incident Coordinator Reports: Director of Quality Assurance Location: Main Office Position Summary: Oversee practices to ensure that procedures enforce programs and service departments to comply with the requirements as it pertains to New York. • Ensuring that administrative responsibilities pertaining to incident classification, reporting, central state systems and committee reviewing procedures are according to policy and OPWDD regulations., • Ensuring that all incidents are reported to require state offices and notifications., • Ensuring that all updated regulations pertaining to incidents are applicable and officially relayed to employees., • Ensure all incidents are reported, managed and reviewed according to New York State regulations., • Investigate and/or oversee investigations to ensure they are conducted according to regulations, that they are conducted thoroughly and with integrity., • Oversee and foster that the incident management process fosters the reporting, investigating, reviewing, correcting, and/or monitoring certain events or situations are to enhance the quality of care provided to persons with developmental disabilities, to protect them (to the extent possible) from harm, and to ensure that such persons are free from abuse and neglect., • Administrate the monthly review Incident Review requirements for New York, • Monitor incident corrective measures to minimize the potential for recurrence of the same or similar events or situations., • Enforce prompt reporting of the incidents, events and situations to ensure that immediate steps are taken to protect persons receiving services from being exposed to the same or similar risk., • Ensure the reporting of certain events, situations and incidents are in an orderly and uniform manner facilitates in order to identify trends, whether within a facility or class of facilities, by one or more agencies, or on a statewide basis, which ultimately allows for the development and implementation of preventive strategies., • Complete annual trend reports for NY., • Maintain and oversee incident reporting records are secured, safe and maintained in an organized manner., • Ensure that all required incident review minutes’ documentation is maintained and completed according to state regulation., • Liaison between the Quality Assurance Specialist and administration as it relates to incident managing compliance., • Ensure that all relevant employees and departments are aware of incident management practices for adequate implementation,, • Ensure all incidents are classified correctly according to respective NY regulations., • Ensure the agency implemented the policies and procedures that are in conformance with OPWDD 624/625 regulations to address (i) reporting, recording, investigation, review, and monitoring of reportable incidents and notable occurrences; (ii) identification of reporting responsibilities of employees, interns, volunteers, consultants, contractors, and family care providers., • Advise employees related to incident reporting and management according to state regulations and agency policy., • Generate quarterly trends incident reports for both state- reportable and state-non reportable incidents., • Prepare Incident Reports which include the description of the incident, the investigation process and findings, the outcome, and recommendations for improvement. Submit reports to designated internal and external parties., • Routinely check/monitor the IRMA/WSIR database to ensure proper closure of incidents, complete and properly labeled packet submissions while also ensuring these databases are up to date per regulatory requirements., • Ensuring data entry is completed in the IRMA database per regulatory requirement., • Routinely check/monitor the incident email and address/respond accordingly to any inquiries within a timely manner., • Being on-call for any incident purposes and ensure all regulatory state requirements related to incidents are followed and adhered to., • Ensure IRC committee meetings are scheduled and are conducted per state regulation and agency policy., • Completing monthly IRC meeting minutes until closure for incidents and ensuring these minutes are entered and up to date in the IRMA database. A Master's degree in a related field may be substituted for 2 of required experience. Job Description JOB SUMMARY: The Clinical Case Management Specialist is a mental health professional who reports directly to the Program Director. This position is primarily clinical and consultative and provides services to the Adult Home Supported Housing Program. The Clinical Case Management Specialist will ensure a smooth transition for Adult Home Residents moving into Supported Housing via the Impacted Adult Home Initiative//Settlement. Attend care coordination meetings, collaborate with ICL in-reach staff and external care coordinators, assess consumer needs, and discuss/advocate for needed services. Maintain adherence with settlement agreements regarding consumers moving into ICL units. Travels to/visits consumers' residences or apartments to provide counsel and assistance to help the consumers, within their capabilities and interests, maintain the most significant degree of independent living.
