Job Description PRIMARY FUNCTION/PURPOSE The Social Services Supervisor is responsible for overseeing a team of Case Managers to ensure the support and resources necessary to promote self-sufficiency and independent living skills for families’ w/children residing in temporary shelter are being provided in accordance with CRF, DHS and OTDA standards. This shelter is located in Long Island City, NY. (Queens) PRIMARY RESPONSIBILITIES • Ensure document folder in CARES (VDP, Client rules and regulations, Crib acknowledgment, ACS Video, etc.) is completed and documented immediately upon client arrival, • Provide field orientation and on the job training to Case Managers, • Create and execute plans for meeting contracted ILP completion and placement goals, • Ensure all Case Managers complete mandated CRF, DHS, and OTDA tasks in accordance with prescribed timeframes and mandates, • Ensure Case Managers are providing consistent services in accordance CRF, DHS and OTDA policies and procedures, • Monitor Case Managers work activities via CARES reviews, physical chart reviews, and spreadsheets, • Conduct individual and group supervisions with Case Managers. ESSENTIAL JOB FUNCTIONS: Through the employee’s own efforts, the employee accomplishes the following essential functions: • Supervisor will have a limited client caseload at the shelter., • Organize and conduct workshops based on client needs at the facility., • Supervisor collaborates to develop programming to address clients’ needs related to housing, substance abuse treatment, mental health and/or employment., • Supervisor provide crisis management and interventions with clients, ensure teamwork and ensure consistent services in accordance with program policies and procedures., • Supervisor responsible for Client Engagement also develop workshops for client engagement, conduct field visits, plan programming to increase community and citywide resources and incentives for client participation., • Responsible for retaining assigned qualified staff and ensures professional growth and development., • Reviews treatment/recovery plans and progress notes, written and electronic, to ensure that clinical services are appropriate with the individual client’s level of care and needs and appropriate with the diagnosis., • Communicates frequently with varied referral sources to maintain working and problem-solving relationships with medical, psychiatric, social services, parole, criminal justice, labor and other resources for client recruitments., • Work from a strength based, family focused, client centered perspective and identify and fortify the strengths that each family has., • Assist families with specific stressors associated with living in a shelter., • Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services., • Aid families in understanding the full range of benefits to which they are entitled and assist with accessing said benefits., • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being., • Serve as agency/program liaison to community agencies and/or groups., • Provide clear reports that capture key findings to include provider areas of strength, and recommendations for improvement., • Ensure that all case management is documented in database system, Cares., • Attend professional development trainings., • Other duties as deemed necessary by the Program Director and/or funding source DHS., • The Supervisor of Social Services will serve as the backup for the Director of Social Services. Job DescriptionTitle of Position: Social Service Supervisor FLSA Status: Exempt Department: Transitional Housing Reports to: Program Director Salary: Up to $64,735.32 per year Positions reporting to this position: Case Managers Purpose of the Position: The Social Service Supervisor is responsible for ensuring Case Managers working in shelters provide quality services to families in accordance with all the Department of Homeless (DHS) services, the NYS Office of Temporary Disability Assistance (OTDA), and Win requirements. The Social Service Supervisor will provide supervision to Case Managers using Evidence Based Practices (EBP), Motivational Interviewing (MI), Trauma Informed Care (TIC), and boundary setting. Responsibilities: * Provide comprehensive case management including individual and group counseling services and other appropriate support services to clients in the absence of the Case Managers. * Supervise Case Managers and provide coaching, training, and staff development as needed. * Manage the shelter’s multidisciplinary team. * Champion the use of Evidence Based Practices (EBP) in all aspects of Win services. * Use and promote EBP, MI, MIC, and TIC to exceed program goals. * Assume responsibility for Program Services in the absence of the Program Director. * Participate in DHS daily housing metrics calls. * Provide crisis intervention, oversee day-to-day case management services and ensure weekly case conferences. * Ensure all Case Managers conduct an initial needs assessment with new families within 48 hours of arrival at the shelter and begin to develop an Individualized Living Plan (ILP). * Participate in Child Safety Conferences and provide information as a Mandated Reporter to the Administration for Children’s Services (ACS) as needed. * Ensure Case Managers are provided with orientation and training related to their responsibilities in maintaining the Uniform Client Chart. * Ensure Case Managers maintain current and accurate confidential case records with all required documentation. * Audit the Client Assistance and Rehousing Enterprise System (CARES) database, review essential documentation, and monitor chart maintenance for OTDA, DHS, and Win audit readiness. * Work with community service providers to ensure clients receive essential services including mandated substance abuse treatment and mental health services. * Attend regularly scheduled CARES training sessions to obtain DHS updates. * Develop and conduct client activities/workshops. * Schedule and attend client monthly house meetings. * Provide feedback to the Program Director to ensure resources are available and the goals of the program are achieved. * Prepare accurate and timely reports required by funders and Win. * Perform other related duties as needed. Essential Functions: * Must be able to work one evening weekly, one Saturday a month, and holidays. * Must be able to ascend/descend stairs in the building where client families reside. * Must be able to escort clients to appointments as needed. Qualifications: * Commitment to Win’s mission, vision and values. * M.S.W. or a related degree required with 3 to 5 years of experience in family and/or single adult residential counseling services. * At least one year of supervisory experience a plus. * Familiarity with EBP, MI, and TIC. * Must be able to work with at-risk populations including domestic violence survivors, clients with medical and mental health issues, and the chronically homelessness. * Must be able to work effectively with a diverse staff. * Excellent organizational and communication skills, written and verbal. * Must be able to work effectively in a team environment * Familiarity with entitlement systems and procedures. * Working knowledge of substance abuse issues a plus. Job Description PRIMARY FUNCTION/PURPOSE: The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The Client Care Coordinator will enhance existing services to include a thorough assessment of clients’ long-term goals at intake, and collaborate with the case management staff to set achievable goals while establishing linkages within the community to ensure the client’s successful progress towards independent living. This individual will work with clients at our Lighthouse location in Long Island City, NY. MINIMUM QUALIFICATIONS: • Must be a Licensed Master Social Worker (LMSW) in the state of New York upon hire or obtain licensure within three (3) months of hire, and have a master’s degree in Social Work from an accredited school of social work, • At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services is strongly preferred, • Knowledge and experience working with diverse cultures and ethnicities, • Expertise in strengths-based, solution-focused, and family-centered practice, • Experience facilitating groups for adults, • Proficiency in Spanish and/or French a plus, • Excellent written and verbal communication skills, • Excellent interpersonal skills and computer literacy in Microsoft-Office are required PRIMARY RESPONSIBILITIES: • Complete a comprehensive bio-psychosocial assessment with each family to understand strengths & service needs, • Provide clear written reports that capture family assessment findings & recommendations, • Assist families that are homeless as they navigate multiple systems & cope with the stressors & anxiety induced by homelessness, • Provide outreach services, crisis intervention, risk assessment, safety planning, & psycho-education for families, • Prioritize & conduct unit visits to the high-risk families that are in-shelter to coordinate safety planning efforts in the household, • Prepare children & parents to accept services, • Collaborate with ACS and/or prevention services agencies, and participate in conferences to advocate on behalf of the family, • Make appropriate referrals and facilitate linkages between mental health providers, government agencies, and other related community–based services as needed, • Serve as agency/program liaison between government agencies, community–based organizations, and/or groups, • Improve access to mental health services for families in-shelter by facilitating in person or telehealth services, • Enhance delivery & coordination of mental health & related services, • Work effectively as part of a multi-disciplinary team of service providers to enhance engagement and ensure quality service, • Confer and consult with professional & technical personnel in implementing a multidisciplinary approach to client care & well-being, • Deliver enhanced mental health services: conduct biopsychosocial assessment, provide on-site mental health services (short counseling & brief intervention services), coordinate clinical treatment (clinical care coordination) referral, as well as linkage to care (medication management or psychotherapy) & follow-up, • Promote in-person or telehealth usage by utilizing H+H Express Care & other telehealth service utilization services via other hospitals & mental health providers of the client’s choice, • Utilize DHS Referral Basics: match the need as assessed through the BPS as soon as possible, assist client in scheduling, provide reminders before the appointment, coordinate transportation to the appointment, facilitate access to teletherapy, and provide a warm hand off when possible, • Follow up on referral provided to the client: confirm the client was able to attend, how the session went, and address any concerns or questions the client might have; if a client did not attend assist with rescheduling & addressing any barriers, • Utilize motivational interviewing if a client does not want to participate or be linked to treatment; attempt to understand your client’s perspective or misconceptions, remove any barriers, and attempt to re-engage, • Provide therapeutic groups (music, art, general support groups for caregivers or children); facilitate group work to address family issues, enhance life skills, etc., • Participate in family meetings with case management & housing staff to discuss planning efforts & progress, • Assist clients with completing applications for supportive housing and accompany residents to supportive housing interviews when necessary, • Upon linkage to permanent housing, complete and discuss exit summary with family, and provide the family with referrals and supportive services identified in new community, • Utilize fact sheets, desk guides, and other material resources in delivering mental health services, • Complete all documentation in a timely manner;
