URAM provides the healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible to perform their job practicing good risk management, quality assurance, and delivering excellent patient experience. Under the supervision of the Medical Director and in accordance with established policies and procedures, the Physician delivers compassionate, high quality comprehensive primary care to homeless, underserved, chronically ill populations including those with diagnoses of HIV, Hepatitis C, SUD, Diabetes, Heart Disease and other comorbidities. The Hospitals are part of NYC Health + Hospitals. Jacobi is a 457-bed, Level I Trauma Center and NCB has 213 licensed beds. The hospital maintains a strong academic affiliation with Albert Einstein Medical College to maintain its high healthcare delivery standards. Our Rheumatology Practice has 1,700 unique active patients between Jacobi Medical Center and North Central Bronx Hospital with 52 patients receiving routine biologic infusion therapy. Job Description POSITION DESCRIPTION The Medical Director of Behavioral Health (Psychiatrist) is responsible for the general oversight and overall direction of Behavioral Health Services at Harlem United. Essential Functions: • Collaborate with residents in the development of client-centered, recovery-oriented service plans that identify short and long-term goals., • Provide integrated counseling and case management services that effectively engage residents and promote health and wellness, personal growth, and housing stability., • Utilize a harm reduction approach to support a diverse caseload of formerly homeless individuals around issues of mental health and substance use., • Make referrals and serve as a liaison with various care coordination agencies, health, and mental health providers, substance use programs, and employment providers to secure services., • Actively engage residents through regular apartment visits, community outings, and escorts to appointments as needed., • Develop safety plans for residents and work with the team to de-escalate clients and collaboratively manage crises as they arise., • Assist in the development and implementation of groups and programs that are appropriate for the resident's needs and foster an inclusive, respectful community., • Develop and maintain confidential records for each resident according to the mandated schedule documenting all relevant interventions and assessments as well as correspondence with collateral services., • Collaborate with the Leasing and Compliance team to ensure housing stability., • Participate in staff and clinical meetings and individual supervision to ensure best practices in the performance of role responsibilities., • Participate in a robust training program that includes training in evidenced-based models to ensure the highest level of service is provided to clients. Required Education and Experience: • High school diploma, HSE or GED is required., • A minimum of 2 years' social services experience is required., • Possession of a valid Food Handler’s License, or the ability to obtain one within six (6) months of hire, is required., • Bi-lingual Spanish/English is preferred but not required., • Ability to work independently as well as a member of an interdisciplinary team., • Highly organized and able to manage a diverse caseload of clients with diverse needs., • Able to work well under pressure including in crisis situations and able to be flexible with work schedule when necessary., • Track record of proactive, strength-based work with individuals and families., • Experience with ETO, or other electronic case management systems., • Experience working with evidence-based models, specifically Motivational Interviewing, is preferred. BronxWorks is seeking one Case Manager to join a diverse and enthusiastic team at Park Haven, a supportive housing program located in South Bronx. Park Haven provides on-site case management services and permanent housing to 50 formerly homeless households, consisting of both single individuals and families with children. The ideal candidate will understand the Housing First model and will work comprehensively with their caseload to work on various tasks that include, but are not limited to, conducting home visits, escorts in the field to various appointments, and completing all necessary documentation in a timely manner. RESPONSIBILITIES • Maintaining a caseload of 12-18 households (individuals and families) with chronic homelessness history., • Monitor and document client progress towards Treatment plan goals., • Conduct regular case conferences with clients and service providers as needed., • Conduct monthly home visits for caseload and field work with clients when needed., • Assist with managing and directing all walk-in clients., • Maintain familiarity with agency and city resources available to clients., • Document progress notes for all encounters and services provided to clients., • Participate in regular meetings, trainings and weekly supervision with supervisor and other support staff., • Completing ongoing Treatment Plans and Assessments for caseload., • Coordinate and connect clients to a variety of medical, mental health and behavioral health physicians and services., • Assessing ongoing eligibility for various entitlements and resources for caseload., • Meeting monthly reporting requirements including timely documentation and engagement with full caseload., • Assisting clients with Representative Payee services including budgeting and financial education., • Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment., • Bachelor’s degree in social work or another related field of study., • One year of case management or relevant work experience preferred., • Engagement and detailed oriented skills are essential., • Proficiency in Microsoft Office suite and other standard business technology is required., • Strong oral and written communication, time management and organizational skills are necessary., • Strong writing and oral communication skills., • Strong presentation skills and ability to represent the agency at functions and meetings., • Ability to collect and analyze client demographics and outcome., • Excellent organizational and teambuilding skills., • Ability to use a computer for prolonged periods., • Ability to occasionally lift and/or move up to 10 pounds., • Ability to stand, walk, or sit for long periods of time., • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork., • Ability to bend and retrieve objects and/or documents., • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation., • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
