Program Description: Funded by the New York City Department of Health and Mental Hygiene (NYC DOHMH), the Harlem Health Advocacy Partners (HHAP) program aims to improve the health of NYC residents by supporting access to health and social services to which they are entitled, providing health coaching to individuals to increase self-efficacy for healthy behavioral changes and disease management, building community capacity to seek and/or create healthy conditions and acceptable services through advocacy to government and other service providers, and providing Group Wellness Activities and high-quality educational workshops. Summary of the Position: The Community Health Worker (CHW) Supervisor oversees a team of five Community Health Workers who will provide health coaching services, community engagement and outreach activities, advocacy campaign events, peer support groups, group wellness activities, and health educational workshops to NYCHA Residents in East & Central Harlem who are diagnosed with poorly controlled asthma, hypertension, and diabetes. The CHW Supervisor will provide direct supervision to Community Health Workers, including quality assurance of individual and group health coaching and wellness promotion activities. The Supervisor works collaboratively with program leadership and peer supervisor to support program consistency, shared learning, and alignment with organizational goals, while maintaining autonomy in the day-to-day supervision and development of their assigned team. The CHW Supervisor will monitor participant transitions with the collective support of the HHAP Team including escalation and participant graduation from the program. In collaboration with HHAP leadership and the peer supervisor, The CHW Supervisor will conduct case reviews, case conferences, and outreach/engagement discussions with all Community Health Workers to provide feedback, share best practices, teach through challenges, and ensure all program participants are making progress towards their health goals. The CHW Supervisor will share responsibility for overall program success and activities, including Health Coaching and Community Engagement efforts. The CHW Supervisor will work with the peer supervisor to monitor Health Coaching activities for the team and collaborate with them on best practices, changes, or concerns. The CHW Supervisor will execute health coaching plans, community outreach plans, monitor community engagement activities, and organize community campaigns with the collective support of the peer supervisors, Program Manager, and the HHAP team. The Supervisor will participate in shared planning of event details, and continuous quality improvement efforts with peer supervisors to strengthen consistency and effectiveness across CHW teams. Specifically, the Community Health Worker Supervisor will: Overall Staff Management and Oversight: • Provide daily supervision, guidance, coaching, and professional development to all reporting CHW staff, as well as support to the entire CHW team, including supervisory coverage for the peer supervisor when they are not available., • Manage HR forms, concerns, and requests for assigned staff., • Monitor staff and team performance, including tracking and monitoring of case documentation, outcomes, and progress towards goals for assigned staff. Complete the annual performance review process., • Work with HHAP Program Manager and peer supervisor to set standard and individualized goals for CHWs., • Assign and monitor caseloads to assess capacity, volume, and productivity., • Review health coaching assessments, workplans, participant action plans, and other case documentation to ensure participants are receiving appropriate coaching and that documentation is accurate and timely; implement strategies in coordination with peer supervisor and Program Manager to address quality assurance needs as appropriate., • Ensure unit is compliant in maintaining updated participant records through electronic case management system (Salesforce)., • Training: Work with the HHAP Program Manager and peer supervisor to implement CHW training, including ensuring compliance with mandatory trainings and identifying resourceful trainings for the CHWs. Support development and maintenance of program training materials, templates and guides, protocols, and CHW resources on SharePoint., • Outreach and Wellness Activities: Collaborate with the HHAP leadership team to establish and maintain an annual outreach and health coaching/wellness activity plan detailing planned activities for the program. Support establishment and coordination of group wellness activities, including partner-run activities., • Scheduling: Act as day-to-day point person for assigned CHW’s schedules and managing the schedule of assigned activities at the developments. This includes assigning CHWs to activities, working with the Program Manager and peer supervisor to maintain the alternate coverage during absences, and covering for peer supervisor and Program Manager in their absence., • Partnership/ Relationship management: In collaboration with HHAP leadership, assign CHWs to NYCHA developments and monitor their progress in scheduling activities at those developments. Maintain and develop partnerships with development contacts and community-based organizations and support CHWs in developing or strengthening relationships with developments., • Advocacy campaigns: In collaboration with HHAP leadership and CHWs, support development and implementation of community organizing campaigns annually. Support assignments of campaign tasks to CHWs., • Support and monitor CHWs on-site at the developments and in the community regularly, provide guidance and oversight to ensure program standards are being met during outreach, health coaching, workshops, and wellness activities., • Complete all required training and maintain current knowledge of information relevant to the project., • Provide information in a manner that is culturally and linguistically appropriate to the needs of the population., • Bachelor's degree in Community Health, or related area plus 2 years of supervisory experience, or equivalent combination of education and experience., • Prior experience in partnership building, community organizing, and network coordination is beneficial., • Experience in case management with highly marginalized populations, particularly those with health, mental health, and substance abuse issues., • Dedicated to helping improve the lives of disenfranchised and marginalized communities., • Experience working and/or living in East or Central Harlem or within NYCHA preferred., • Bilingual in Spanish or Chinese Mandarin preferred., • Proficiency in Microsoft Word, Excel, and PowerPoint., • Flexible team player with demonstrated ability to work collaboratively and respectfully both internally and externally within collaborative or multi-layered team environments and engage in consensus-based decision-making., • Strong ability to learn tasks and new topics quickly; apply critical thinking skills., • Highly organized, detail-oriented, able to work independently and shift tasks based on priorities., • Strong relationship-building, interpersonal, and organizational communication skills, including listening, written, oral, and public speaking., • Ability to thrive in a work environment that is respectful, goal-oriented, and focused, and yet is dedicated to achieving work-life balance., • Ability to embrace diversity; possess good people and cultural competency skills., • Ability to lift and carry personal equipment (e.g. laptop) and outreach materials as needed., • Ability to travel around New York City’s five boroughs., • Hybrid Work Schedule., • Generous Paid Time Off and Holidays., • An attractive and comprehensive benefits package including Medical, Dental and Vision., • Flexible Spending Accounts and Commuter Benefits., • Company Paid Life Insurance and Disability Coverage., • 403 (b) + employer matching and discretionary company contributions., • College Savings Plan.
