The role coordinates, rather than replaces, the clinical accountability of medical staff, nursing leaders, emergency care, and neurointerventional teams. Essential Duties and Responsibilities — System Leadership • Own and maintain a single system stroke program workplan, governance calendar, milestone tracker, risk register, and site-specific certification readiness dashboards., • Lead multidisciplinary stroke steering and site workgroups; verify case-volume, provider credentialing, and other TSC-specific criteria directly against current DNV requirements.Clinical Quality, Data, and Performance Improvement, • Build a standardized stroke registry/abstraction process with reliable case identification, data validation, and site-level and system-level dashboards., • Monitor stroke-alert performance, imaging and treatment times, thrombolysis and thrombectomy process measures, transfers, contraindication documentation, complications, outcomes, and guideline adherence, as applicable to each site., • Lead timely multidisciplinary reviews of treatment delays, missed opportunities, adverse events, and deviations; Job DescriptionDescription: Position Summary Under the direct supervision of the SVP of Behavioral Health, the School-Based Behavioral Health Provider (BHP) is responsible for providing trauma-informed behavioral health services at the Zufall Health Center sites with a focus on our school-based health center. Zufall Health follows the Primary Care Behavioral Health model of integrated care. The BHP provides direct services to patients, families, and groups. They provide brief behavioral health intervention to improve patient health outcomes as well as individual and family counseling, and engages in crisis intervention. The BHP contributes to the patient care team through huddles, case consultation, patient advocacy, mental health education, and collaborative team meetings with clinical and support staff. They identify opportunities for enhancing integrated Behavioral Health Services in the primary care setting and serve as a resource to the rest of the clinical staff. The BHP will partner with the school to develop relationships and support the students and the community at large. BHP’s working towards clinical licensure will participate in weekly clinical supervision (individual or group) with an LCSW who is certified to provide clinical supervision. Essential Functions, Duties and Responsibilities • Support the school community by:, • Partnering with students and families, • Developing collaborative relationships with school staff to ensure effective care, • Assisting in prevention efforts at the school including classroom lessons, groups, and community workshops, • Participating in school activities such as back to school night, • Work in collaboration with the primary care providers to recognize and treat mental health disorders and psychosocial problems., • Work with the primary care team to treat and manage patients with chronic emotional and/or physical health problems., • Complete a Comprehensive Assessment and Treatment Plan based on patient's history and relative to the presenting problem. Follow established protocols and methodologies., • Collaborate with patients in the development of treatment plans and measurable goals and outcomes., • Review cases regularly with clinical supervisor., • Maintain patient records well documented and in a timely fashion., • Participate in crisis intervention as response for patients dangerous to self/others/property., • Lead group sessions and activities, and develop curricula for specific initiative (for example, Healthy Lifestyles)., • Conduct pre- and post-intervention assessments and patient surveys to evaluate effectiveness of programs., • Participate and present in care conferences, patient groups, educational programs for staff and for patients, staff meetings, staff trainings and other venues as assigned., • Represent Zufall Health Center at community service organization meetings, behavioral health agencies and collaborating entities., • Participate in outreach and community educational activities as part of the Care Team as assigned by the SVP of Behavioral Health and/or Chief Medical Officer., • Participate in in-service educational programs and continuing education., • Teach and train other health center staff and students as needed., • Follow established departmental policies and procedures regarding quality assurance, safety, environmental and infection control standards. Assist in the development of relevant policies and procedures as the department and services evolve., • Participate in medical/clinical staff meetings and general staff meetings., • Ensure patient confidentiality under HIPAA regulations., • Perform other duties as assigned. Job DescriptionDescription: Reporting to the SVP of Nursing and Clinical Operations (SVPNCO) in conjunction with the Director of Prenatal Program and the Prenatal and Post Partum Care Nurse, the Prenatal Case Manager assists prenatal patients and their families in accessing appropriate clinical and social services. The Prenatal Case Manager is a key member of the Care Team and focuses on needs and barriers (such as financial, cultural, language, insurance) that prevent patients from achieving healthy outcomes. Travel is expected to be local with required attendance at some community meetings. Essential functions, Duties and Responsibilities • Works as part of the Care Team to identify prenatal patients in need of services and communicates with provider as to progress, • Meets initially with each prenatal patient and follow up visits as required by patient needs or referrals, • Keeps an active problem list and tracks patients that have been referred. Documents level of services provided and discuss with MCHI PC on a weekly basis. Updates Plan of Care as needed, • Follows up on missed appointments and reschedules as appropriate. Contacts prenatal patients that are overdue for visits, • Enters interventions and follow up plans in electronic medical record-eCW, • Assists prenatal patients with arrangements for transportation, translation, and other barriers to attending appointments, • Assists prenatal patients with completion of medical, prescription, service forms and with requests to other providers and agencies, • Assists patients with applications to charity care at Clara Maas Medical Center, Emergency Medicaid and PW Medicaid, NJ Family Care, WIC and other benefits or insurances as appropriate, • Completes PRAspect initial file entry for every patient required by the State of NJ, • Updates PRAspect file as insurance changes from PEPW, Medicaid, HMO, 3rd trimester, and final birth data required for Birth Certificate production, • Facilitates referrals for Zufall Health Center providers to refer prenatal patients into behavioral health and enabling services, including referrals to outside agencies and programs. Facilitates interchange of pertinent clinical information as per HIPAA guidelines to the Care Team and outside providers involved in the care of the patient., • Facilitates first Newborn visit with Zufall Health Providers, • Identify community resources and services that address barriers encountered by prenatal patients and their families and assist patients in accessing such services, • Acts as advocate for patients and their families with community organizations and other agencies, • Conducts Spanish speaking classes relative to Prenatal patients as trained/directed by Prenatal Coordinator. (Breastfeeding, Labor & Delivery), • Educates patients on acquiring a Breast pump through Insurance or WICRequirements:, • Working knowledge of medical terminology, • Proficient, accurate and detailed documentation, • Excellent knowledge of available community services and resources, • A working knowledge of prenatal care needs and possible barriers, • Excellent communication and interpersonal skills, • Ability to assist patients through adverse pregnancy outcomes with empathy, • Ability to work as part of a care team, • Bilingual English/Spanish required, • Ability to communicate well with patients, staff and outside providers and agencies, • Ability to maintain high level of confidentiality, • Assist supervising/training AmeriCorp members/volunteers, • Working knowledge of Zufall Health Center policies and procedures, mission and values, • Some travel may be required between sites.
